Study On Professor Ma Hongbin’s Experience And Medication Law In The Treatment Of Renal Failure In Chronic Kidney Disease From Stage Two To Four

Objective: Analyze and summarize the medication experience of Professor Ma Hongbin in the treatment of chronic renal failure(CKD2-4),to guide the clinical treatment of chronic renal failure,and to improve the clinical effect.Methods: Collected cases of Chronic kidney disease Stages 2 to 4 treated by Professor Ma Hongbin from September 2017 to December 2019 through clinical follow-up,and applied the software called "Traditional Chinese Medicine Inheritance Assistance Platform(V2.5)" to establish "Database on Ma Hongbin’s treatment of chronic renal failure(CKD2-4)" for data entry.The software is used to calculate the drug frequency and times,summarize the high-frequency drug pairs based on association rules,analyze the rule of prescription based on the complex entropy clustering algorithm,and summarize the drug core combinations and new prescription combinations,etc.Generalize the academic concepts of Professor Ma,which are formed through the accumulation of clinical practice over many years.By this way to summarize his theoretical concept of chronic renal failure,and analyze his medication experience at a deeper level.Results:1.The clinical symptoms of chronic renal failure are mainly include fatigue,anorexia,dizziness,and nausea,which may be accompanied by chest tightness,palpitations,vomiting,pale complexion,and pruritus.Professor Ma believes that the pathogenesis of traditional Chinese medicine for this disease is "the essence of deficiency and marked by excess",which is based on the deficiency of kidney,spleen,and stomach,and the causes of wind,dampness,heat,stasis and turbidity.Chronic renal failure is divided into six types: mix of wind and humidity with heat and stasis,internal disturbance of phlegm and heat,spleen deficiency and damp heat,qi deficiency and damp resistance,kidney deficiency and damp heat,and deficiency of qi and Yin.The prescriptions uses Zhao’s Formula,Huanglian Wendan decoction,Nephritis Siwei decoction,Xingshi Buqi Yangxue soup,New combination of Bixiefenqing yin,and Shenkangling plus or minus.The use of drugs are filled with the tonic deficiency,clearing away heat and dampness,cooling blood and expelling wind,removing blood stasis and clearing stasis,and regulating ventilator.In clinical practice,tonic deficiency andclearing heat are used throughout the treatment,and they are good at the use of the pair drugs.Pay attention to the relationship between heart,liver,san jiao and kidney,spleen and stomach.Emphasizes the treatment of the spleen and kidney,and take care of the stomach.2.Based on the statistics of medication frequency,the top ten drugs used by Professor Ma in the treatment of Chronic kidney disease Stages 2 to 4 are Poria cocos,Liquorice,Angelica,Astragalus,salvia miltiorrhiza,Shengdihuang,Codonopsis,Atractylodes macrocephala,Baicao root,and Ophiopogon.The first six categories of high-frequency drug efficacy classification are: heat-clearing drugs,tonic-deficiency drugs,diuretic and damp-drenching drugs,blood-activating and blood-stasis removing drugs,antipyretic drugs,and wenli drugs.Based on the statistics of the four qi,five flavors and meridian of the drug,cold,flat,warm,sweet,bitter,and astringent.spleen,kidney,and liver are attributes of common medicinal odor.Analysis the formula rule based on the association rules,the first eight drug combinations are Astragalus-Poria,Liquorice-Poria,Angelica-Poria,Salvia-Poria,Shengdihuang-Poria,Shengdihuang-Angelica,Baizhu-Poria,Astragalus-Angelica.There are fourteen results on the association rule analysis of drug combination.Based on the improved mutual information method,twenty-six correlation drug pairs were obtained.Fourteen core combinations are obtained from the analysis of complex system entropy clustering.Seven new prescriptions were analyzed based on unsupervised entropy hierarchical clustering.Conclusion:1.Professor Ma summarized the pathogenesis of chronic renal failure as "the deficiency of kidney,spleen,and stomach,and the causes of wind,dampness,heat,stasis and turbidity.The main types of syndromes are divided into six: mix of wind and humidity with heat and stasis syndrome,phlegm-heat internal disturbance syndrome,spleen deficiency and damp heat syndrome,qi deficiency and damp resistance syndrome,kidney deficiency and damp heat syndrome,and qi and yin deficiency syndrome.The recipe chooses Zhao’s Formula,Huanglian Wendan decoction,Nephritis Siwei decoction,Xingshi Buqi Yangxue soup,New combination of Bixiefenqing yin,and Shenkangling plus or minus.Attach importance to the pathological points of "deficiency" and "heat",tonics and heat-clearing drugs are used throughout the treatment.Emphasis the combination of syndrome differentiation and disease differentiation.Take the advantages of traditional Chinese medicine and Western medicine.Pay attention to the conditioning of kidney,spleen,stomach and heart,liver,and san jiao,and focus on the treatment of spleen and kidney.2.Comprehensively summarize the experience and laws of drug use of Professor Ma in the treatment of Chronic kidney disease Stages 2 to 4 through the "Traditional Chinese Medicine Assistance Platform",so as to guide the clinical treatment of chronic renal failure and improve the clinical efficacy.
