Changes Of Coagulation Index In Patients With UC And Its Clinical Guiding Value In Disease Activity

Objective:UC patients and healthy controls the related parameters of blood coagulation,platelet(PLT),platelet and lymphocyte ratio(PLR),average blood platelet volume(MPV),platelet than product(PCT)and platelet volume distribution width(PDW),prothrombin time(PT),prothrombin activity(PA),fibrinogen(FIB)quantitative,thrombin time(TT)and activated partial prothrombin time(APTT)difference;To analyze the indexes with differences between the UC group and the control group and significant differences in disease stages of different activities,the ROC curve was plotted according to the Mayo score in order to find the indexes with application value for the activity of UC.Method:In July 2017 to July 2019 to our hospital for diagnosis and treatment,and conforms to the diagnosis and treatment of inflammatory bowel disease the consensus of opinion "(in 2018,Beijing)[1](shall be formulated by the Chinese medical association digestion neurology,IBD group)in the diagnosis of ulcerative colitis based on 127 UC patients as the research object,according to the improvement of the Mayo scoring system(table 3.1)assessment of disease activity,in which 2 cases is in remission,26 cases at moderate activity,72 cases in moderate activity,27 cases of severe activity.Meanwhile,58 healthy control subjects(age and gender matched)who underwent physical examination in our hospital during the same period were included.The differences of PLT,MPV,PDW,PCT,PLR,PT,PA,FIB,TT and APTT between UC patients and healthy control group were observed.Analysis and the above indexes in UC group and control group differences and activities in different activities on the stage there is obvious difference of index,by Spearman to select indicators and Mayo scored and correlation analysis between c-reactive protein and blood sedimentation,and through the ROC curve to predict the PLT,PLR,PT,FIB in UC application value of disease activity.Result:(1)PLT,PCT,PLR,PT and FIB in the UC group were significantly higher than those in the control group(266.00vs215.00,0.25vs0.22,170.4vs117.1,12.95vs12.12,3.56vs2.81).P<0.01),MPV and PA in the UC group were significantly lower than those in the control group(9.20vs9.85,86.92vs96.81).P<0.01),PDW and TT were lower than the control group(15.5vs15.8,17.1vs18.55).P<0.05),there was no significant difference in APTT between the control group and the control group(33.8vs32.75,P>0.05).(2)FIB in the mildly active group of UC patients was lower than that in the moderately active group(P<0.05),TT was higher than that in the moderately active group(P<0.05),and there were no statistically significant differences in MPV,PT,PA,PLT and PLR between the mildly active and moderately active groups(P>0.05).MPV and PA in the mildly active group were significantly higher than those in the severely active group(P<0.01),while PT,FIB,PLT and PLR were significantly lower than those in the severely active group(P<0.01).In the moderately active group,PA was significantly higher than that in the severely active group(P<0.01),MPV was higher than that in the severely active group(P<0.05),PT,FIB and PLR were significantly lower than that in the severely active group(P<0.01),and PLT was lower than that in the severely active group(P<0.05).(3)in patients with UC,PT,PLT,PLR and FIB were significantly positively correlated with esr(r=0.344,r=0.463,r=0.323,r=0.721).TT,MPV and esr were negatively correlated(r=-0.227,r=-0.256).P < 0.05);PT,PLT,PLR and FIB were positively correlated with c-reactive protein(r=0.523,r=0.324,r=0.326,r=0.522).P<0.01),PA and MPV were negatively correlated with c-reactive protein(r=-0.443,r=-0.232).P<0.01),TT was negatively correlated with c-reactive protein(r=-0.209).P < 0.05);There was a significant positive correlation between PT,PLT,PLR and FIB and Mayo score(r=0.332,r=0.308,r=0.24,r=0.455).P<0.01),PA and MPV were negatively correlated with Mayo score(r=-0.176,r=-0.152).P < 0.05)and(4)using the Mayo score,by the Mayo score > 2 is divided into the boundary,respectively with PLT,PLR ROC curve drawing,PT,FIB,PLT AUC 0.731,when the cut off value of 241.5,the sensitivity was 63.8%,77.6%(P < 0.01),the PLR AUC is 0.69,when the cut off value of 144.778,the sensitivity was 61.4%,77.6%(P < 0.01),FIB AUC is 0.78,When the cut off value was 3.195,the sensitivity was 70.1% and the specificity was 79.3%(P<0.01),the AUC of PT was 0.683,and when the cut off value was 11.75,the sensitivity was 82.7% and the specificity was 48.3%(P<0.01).Conclusion:1.There was a significant positive correlation between PT,PLT,PLR and FIB and ESR,c-reactive protein and Mayo scores in UC patients in the active phase.PLT,PLR,PT,FIB can be used for disease activity monitoring.2.FIB gradually increases with the aggravation of the severity of UC,which can be used for the preliminary assessment of the severity of the disease.
