| Objective:To investigate the predictive role of Triglyceride(TG)/High density lipoprotein-cholesterol(HDL-C)ratio in the development of coronary collateral circulation in patients with chronic total occlusion of coronary arteryMethod:The patients were selected from the cardiovascular disease diagnosis and treatment center of Bethune First Hospital of Jilin University from January,2016 to December,2018,who were was confirmed by coronary angiography presence of at least one chronic coronary occlusion.According to the inclusion and exclusion criteria,a total of 401 patients were selected,Among them,286(71.3%)were male and 115(28.7%)were female.According to the different levels of Rentrop in CAG,the patients were divided into two groups:good CCC group(Rentrop 2-3)(178 cases)and bad CCC group(Rentrop 0-1)(223 cases).Comparison of general clinical data between the two groupsResult:(1)Compared with the good CCC group, the bad CCC group was more common in men, with a high proportion of previous diabetes history. There were significant differences in left ventricular ejection fraction, total bilirubin, direct bilirubin, indirect bilirubin,triglyceride,high-density lipoprotein cholesterol,Fasting blood glucose,TG/HDL-C ratio between the two groups(P<0.05).Univariate analysis of variance showed that TG/HDL-C ratio was negatively correlated with the grade of CCC Rentrop(TG/HDL-C ratio of grade 0,1,2 and 3 was 2.61±2.03、2.34±1.38、1.65±0.96、1.54±0.96,respectively,P<0.001)(2)After adjusting for hypertension, diabetes, left ventricular ejection fraction, left ventricular diastolic end diameter, total cholesterol,low-density lipoprotein cholesterol,Fasting blood glucose,TG/HDL-C ratio and other factors by logistic binary regression analysis, higher TG/HDL-C ratio(OR 1.964,95%CI 1.534-2.515,P<0.001),lower left ventricular ejection fraction(OR 0.958,95%CI 0.923-0.994,P=0.023),diabetes mellitus(OR 1.513,95%CI 1.001-2.285,P=0.049)were independent positive predictors of CCC dysplasia(3)After ROC curve analysis, the best cross-section point of TG/HDL ratio was 1.38,the area under ROC curve was 0.710(95%CI was 0.657-0.762,P<0.001),the sensitivity of TG/HDL-C ratio in predicting CCC dysplasia was 79%,the specificity was 55%,TG/HDL-C ratio was of medium diagnostic valueConclusion:In CTO patients, high TG/HDL-C ratio is an independent risk factor for CCC dysplasia, which has a high predictive value for CCC development. |
