Background and purpose Primary liver cancer is the sixth most common malignancy and the third most common cause of cancer death.Primary liver cancer includes primary hepatocellular carcinoma(HCC),primary intrahepatic cholangiocarcinoma and mixed forms,among which primary hepatocellular carcinoma accounts for the majority.There are many treatment options for HCC,including radical surgical resection,radiofrequency ablation,interventional therapy,targeted drugs,radiotherapy and other treatment options,but radical surgical resection is preferred.The recurrence rate of HCC was higher within 1 year after surgery,and the higher postoperative recurrence rate of HCC patients was related to preoperative factors.This study was a retrospective analysis of 125 HCC patients who underwent hepatectomy.Independent risk factors for HCC recurrence within 1 year after surgery were identified and analyzed.To provide certain help for the clinical treatment and postoperative prognosis of HCC.The research methods Clinical data of HCC patients who underwent radical hepatectomy in the first hospital of jilin university from October 2016 to October 2017 were collected.The possible risk factors for HCC recurrence within 1 year after radical HCC surgery were listed.These factors include(1)the patient’s basic information: gender,age,drinking history,smoking history,hypertension history,diabetes history,hepatitis B virus infection,hepatitis C virus infection;(2)biochemical data: AFP level,the degree of liver fibrosis,indocylane-green clearance test,AST and ALT levels,and neutrophil-lymphocyte ratio(NLR).(3)tumor characteristics: tumor diameter,tumor number and vascular invasion;(4)operation related data: intraoperative blood transfusion volume.All the above risk factors were analyzed by chi-square test.Univariate factorswith significance of chi-square test were included in binary classification logistic regression analysis to obtain independent risk factors for HCC recurrence within 1year after surgery.The results of the study1.The number of 125 HCC patients with recurrence within 1 year after radicalhepatectomy was 41,with a 1-year recurrence rate of 32.8%.2.All possible recurrence factors were analyzed by chi-square test for single factor,with P≤0.1 as the standard.The results showed that AFP>200ng/ml,degree ofliver fibrosis>F2,tumor number >1,tumor diameter >5cm,vascular invasion andNLR>3 were risk factors for recurrence in HCC patients within 1 year aftersurgery(all P ≤0.1).Gender,age,history of diabetes,history of hypertension,history of smoking,history of alcohol consumption,infection with hepatitis bvirus,infection with hepatitis c virus,AST,ALT and ICG15 were not associatedwith recurrence of HCC within 1 year after surgery(all P were >0.1).3.Univariate analysis results(P≤0.1)obtained by chi-square test were included inbinary logistic regression analysis,with P<0.05 as the standard.The resultsshowed that the degree of liver fibrosis(P=0.003)and vascular invasion(P=0.002)were independent risk factors for recurrence within 1 year after radicalhepatectomy in HCC patients(all P <0.05).AFP>200ng/ml,tumor number >1,tumor diameter >5cm and NLR>3 were not independent risk factors forrecurrence within 1 year after radical hepatectomy in HCC patients(all P >0.05).Conlution The risk factors for HCC recurrence within 1 year after radical hepatectomyinclude: AFP>200ng/ml,degree of liver fibrosis>F2,tumor number >1,tumordiameter >5cm,vascular invasion and NLR>3,according to the analysis of datafrom 125 patients.The degree of liver fibrosis>F2 and vascular invasion wereindependent risk factors for recurrence in HCC patients within 1 year after radicalhepatectomy.
