Objective: In this study,we systematically observed Mu Fang Ji Decoction in the treatment of chronic obstructive pulmonary disease patients with Fei Yu Shui Ting syndrome of COPD,and objectively evaluate its clinical efficacy.At the same time proposed and verified the intervention of Mu Fang Ji decoction in Fei Yu Shui Ting syndrome of COPD The possible mechanism of mucus hypersecretion is related to the work hypothesis related to the abnormal expression of MUC5AC(Mucoprotein,mucin)induced by NE(neutrophil elastase,neutrophil elastase)-related signaling pathways,which aims to treat Fei Yu Shui Ting syndrome of COPD patients provide objective theoretical basis.Methods: In this study,60 patients with Fei Yu Shui Ting syndrome of COPD that met the standard were divided into the combination of the traditional Chinese and western medicine group.the traditional Chinese medicine group and western medicine group according to the random number table method.In 18 cases,42 effective cases were finally collected.The combination of traditional Chinese and western medicine group was given oral administration of Mu Fang Ji Decoction on the basis of conventional western medicine treatment.The traditional Chinese medicine group was treated with oral administration of Mu Fang Ji Decoction,and the western medicine group was treated with conventional western medicine treatment.The treatment duration of all three groups was 10 days.Observe the changes of individual symptom scores and total points of traditional Chinese medicine before and after treatment,and use m MRC and CAT scales to evaluate related symptoms.We observe the percentage of neutrophils and Elisa method are used to detect the levels of serum NE and MUC5 AC before and after treatment.Finally,we statistically analyze these data.Results:(1)Basic situation,the three groups of patients are not statistically significant in terms of gender,age,smoking history,course of disease and so on are at the same baseline,and are comparable(P>0.05).(2)The traditional Chinese medicine single symptom scores and total symptoms score,Compared with before treatment,The scores of cough,sputum,the sense of suppression in the chest,the sense of chest and stomach obstruction,puffiness of the head and face,poor urine in the combination of traditional Chinese and western medicine group were significantly reduced(P<0.01),and the shortness of breath and dyspnea scores were reduced(P<0.05).The scores of cough,sputum,and poor urine were significantlyreduced in the traditional Chinese medicine group(P<0.01),and the scores of the sense of chest and stomach obstruction and puffiness of the head and face were significantly reduced(P<0.05).The scores of cough,sputum,the sense of suppression in the chest,shortness of breath and difficulty breathing in the western medicine group were significantly reduced(P<0.01),and the scores of dull complexion was reduced(P<0.05).Compared with the total score of traditional Chinese medicine symptoms before treatment,the total score of the combination of traditional Chinese and western medicine group,traditional Chinese medicine group and western medicine group all decreased significantly after treatment(P<0.01).(3)Comparison of individual symptom scores and total symptom scores of traditional Chinese medicine between groups before and after treatment,It is statistically significant in sputum,the sense of suppression in the chest,shortness of breath and difficulty breathing,the sense of chest and stomach obstruction,and poor urine(P<0.05),but no statistical significance in coughing,dull complexion,and puffiness of the head and face(P>0.05).Pairwise comparison between groups,the difference in the combination of traditional Chinese and western medicine group is greater than that of the western medicine group in terms of expectoration significantly statistically significant(P<0.01).The difference in the combination of traditional Chinese and western medicine group is greater than that of the traditional Chinese medicine group in the sense ofsuppression in the chest significantly statistically significant(P<0.01).For shortness of breath and difficulty breathing,the difference between the group of traditional Chinese and western medicine is greater than that of the group of traditional Chinese medicine,and the difference is statistically significant(P<0.01).The difference of the group of traditional Chinese medicine is greater than that of the group of western medicine,and the difference is statistical significance of learning(P<0.05).On the sense of chest and stomach obstruction,the difference between the combination traditional Chinese and western medicine group is greater than that of the western medicine group,and the difference is statistically significant(P<0.01).on the poor urine,the difference between the combination of traditional Chinese and western medicine group is greater than the western medicine group,and the difference was statistically significant(P<0.05).The difference between the three groups before and after the treatment of the total score of traditional Chinese medicine symptoms was statistically significant(P<0.05).Pairwise comparison between the two groups,the total score of the combination of traditional Chinese and western medicine group the difference was significantly greater than that of the traditional Chinese medicine group,and the difference was statistically significant(P<0.01).The difference between the total score of the combination of traditional Chinese and western medicine group was greater than that of the western medicine group,and the difference was statisticallysignificant(P<0.05).There was no statistical comparison between the other groups significance(P>0.05).(4)Comparison of CAT and m MRC points before and after treatment in the group,Compared with the CAT and m MRC points before treatment,the three groups all decreased after treatment,and the difference was statistically significant(P<0.05).(5)Comparison of integral difference between CAT and m MRC before and after treatment between the three groups,The difference in CAT integral before and after treatment between the three groups is significantly statistically significant(P<0.01),and the comparison between the two groups indicates that the difference between the combination of traditional Chinese and western medicine is greater than that of traditional Chinese medicine Group,and the difference is statistically significant(P<0.05).The difference between the combination of traditional Chinese and western medicine group is greater than that of the western medicine group,and the difference is statistically significant(P<0.05).There is no statistical significance between the traditional Chinese medicine group and the western medicine group(P>0.05).The difference in the integral difference between the three groups before and after m MRC treatment was statistically significant(P<0.01).The comparison between the two groups showed that the difference between the integrated traditional Chinese and western medicine group was greater than that of the western medicine group,and the difference was statistically significant(P<0.01).There was nostatistically significant difference between the combination of traditional Chinese medicine and western medicine group and traditional Chinese medicine group,and between traditional Chinese medicine group and western medicine group(P>0.05).(6)Analysis of the total therapeutic effect between groups,the effective rate of combined traditional Chinese and western medicine group was 94%.The effective rate of traditional Chinese medicine group was 70%.The effective rate of the western medicine group is 86%.The effective rate of the combination of traditional Chinese and western medicine group is higher than that of the traditional Chinese medicine group and the western medicine group,and the difference in efficacy between the three groups is statistically significant(P<0.05).(7)Comparison of NE and MUC5 AC content and NEUT%,there was no statistically significant difference between NE and MUC5 AC before and after treatment in the three groups(P>0.05).There was no statistically significant difference in the NEUT% of the traditional Chinese medicine group before and after treatment within the group(P>0.05).The difference between the western medicine group and the combination of traditional Chinese and western medicine group was statistically significant(P<0.05).(8)Treatment between groups Comparison of NE,MUC5 AC,NEUT% difference in serum of patients before and after,the difference between NE and MUC5 AC before and after treatment in the three groups was not statistically significant(P>0.05).The differencebetween NEUT% before and after treatment was statistically significant(P<0.05).The comparison between the two shows that the difference between the combination of traditional Chinese and western medicine group is significantly greater than that of traditional Chinese medicine group,which is statistically significant(P<0.01).The difference between the combination of traditional Chinese and western medicine group is greater than that of western medicine group,which is statistically significant(P<0.05).The difference was not statistically significant(P>0.05).Conclusion: The treatment of patients with Fei Yu Shui Ting syndrome of COPD with Mu Fang Ji Decoction alone can significantly improve the symptoms of patients with cough,sputum,poor urine,the sense of chest and stomach obstruction,epuffiness of the head and face,reduce the percentage of neutrophils,and no obvious toxic and side effects were observed.On the basis of conventional western medicine treatment,the addition and subtraction of Mu Fang Ji decoction can be used to differentiate and treat patients with AECOPD.It is better than simple western medicine and traditional Chinese medicine to improve patients’ cough,sputum,shortness of breath and difficulty breathing,the sense of suppression in the chest,puffiness of the head and face,poor urine,and dull complexionsymp toms,are conducive to control the disease,significantly improving clinical efficacy,and have good safety,the expression of serum NE and MUC5 AC in the three test groups showed a downward trend,butthere was no statistical significance,so it has not been confirmed that Mu Fang Ji decoction can treat the airway mucus hypersecretion of Fei Yu Shui Ting syndrome of COPD is related to its interference with the abnormal expression of MUC5 AC induced by NE-related signaling pathways,and further research is needed. |
