OBJECTIVE: To evaluate the efficacy and safety of the collapse method in the treatment of airway mucus hypersecretion during acute exacerbation of chronic obstructive pulmonary disease(AECOPD),to provide a new method for the treatment of AECOPD by the external treatment of Chinese medicine,and to provide a theoretical basis and data support for clinical application.METHODS: A prospective,randomized parallel cohort design was used to screen patients with AECOPD who were admitted to the respiratory department of Jiangxi Provincial Hospital of Traditional Chinese Medicine from 2021.11 to 2022.12.A total of 66 patients who met the enrollment requirements were included,and the enrolled patients were randomly(lottery method)assigned 1:1 to the collapsed group and the control group.The control group received conventional Western medicine treatment and Er Chen combined with San Zi Yang Yin Tang plus reduction orally,while the collapse group received Chinese medicine collapse treatment based on the control group,once a day for 7 days.The basic data of patients and the changes of TCM symptom score,severity of dyspnea,sputum characterization score,pulmonary function,infection index and sputum MUC5 AC content before and after treatment were recorded in detail and analyzed by statistical processing.RESULTS: Six patients were shed,and finally 60 patients completed the study.Through the comparative analysis of all relevant data,it can be seen that: 1.the basic data(gender,age)of patients in the collapse group and control group were statistically compared before treatment,and the differences were not statistically significant(P>0.05),with comparability;2.in the collapse group of 30 cases,there were 0 cases of clinical control,2 cases of significant effect,27 cases of effective,and 1 case of ineffective,with a total effective rate of 96.7%;in the control group of 30 cases,there were 0 cases of clinical control,1 case of significant effect,27 cases of effective,and 1 case of ineffective.The total effective rate was 93.33%.By comparing the effective rate,it can be concluded that the clinical efficacy of the collapsed group was relatively better,and the efficacy of the control group was relatively inferior;3.After treatment,both groups could effectively improve the Chinese medicine symptom score,and the collapsed group was better than the control group,and the difference was statistically significant(P < 0.05);4.After treatment,both groups could not effectively improve the m MRC score,and the difference was not Both the collapsed group and the control group could effectively improve the sputum characterization score after treatment,and both groups improved significantly after treatment compared with before treatment(P < 0.05),and the collapsed group was better than the control group,and the difference was statistically significant(P< 0.05);6.The collapsed group could effectively improve the pulmonary function FEV1 value after treatment(P < 0.05),and the control group failed to effectively improve the pulmonary function FEV1 value after treatment(P < 0.05).The collapsed group could effectively improve the pulmonary function FEV1/FVC value after treatment(P<0.05),while the control group failed to effectively improve the pulmonary function FEV1/FVC value after treatment(P>0.05),and the collapsed group was not better than the control group,and the difference was not statistically significant(P<0.05).Both the collapsed group and the control group could effectively improve the infection indexes(WBC and NEUT%)after treatment,and both improved significantly after treatment compared with before treatment(P < 0.05),and the collapsed group was better than the control group,and the difference was statistically significant(P < 0.05);9.Both the collapsed group and the control group could effectively improve the sputum MUC5 AC content after treatment,and both improved significantly after treatment compared with before treatment(P < 0.05).The difference was statistically significant(P < 0.05),and the collapsed group was better than the control group.Conclusion: 1.Chinese medicine collapse adjuvant treatment for acute exacerbation of chronic obstructive pulmonary disease can effectively relieve patients’ clinical symptoms,improve lung function and infection index,and relieve airway mucus hypersecretion symptoms by reducing MUC5 AC content,which is an effective,economic,safe and feasible treatment mode and can be promoted in clinical application. |
