ObjectiveTo investigate the status of discharge readiness,self-care agency,and health literacy of patients after percutaneous coronary intervention(PCI)and analyze its influencing factors;to explore the correlation among discharge readiness,self-care agency and health literacy of patients after PCI,to provide theoretical basis for improving the readiness of patients discharged after PCI and reducing the readmission rate,so as to explore the mediating effect of self-care ability between health literacy and discharge readiness by stepwise regression.MethodsPatients who underwent PCI in a cardiology department of a third-class hospital in Beijing were selected by the convenient sampling method as the research object,and a questionnaire survey was performed on patients who met the inclusion and exclusion criteria for PCI after May to December 2019.The questionnaire includes: the General Information questionnaire,Readiness for Hospital Discharge Scales,Exercise of Self-Care Agency Scale,and Health Literacy Management Scale.We used SPSS24.0 statistical software to analyze the data.Frequency and composition ratio were used to describe the general information of patients after PCI.Mean,standard deviation were used to describe patients’ readiness for discharge,self-care agency,and health literacy scores after PCI.Univariate analysis(t test,ANOVA),multivariate linear regression analysis of influential factors of patients’ readiness for discharge,self-care ability and health literacy after PCI.Pearson correlation analysis was used to explore the correlation among discharge readiness,self-care agency and health literacy.Results1 The average postoperative discharge score of patients after PCI is(92.43 ± 16.27),the individual status dimension score is(22.76 ± 3.66),the adaptability dimension score is(38.01 ± 8.04),and the expected support dimension score is(31.65 ± 7.03)points.Differences in the overall discharge readiness of patients with different marital status,age,education,monthly household income per capita,place of residence,number of comorbidities,and depression were statistically significant.2 The average post-PCI patient self-care ability score is(108.99 ± 28.29),the self-concept dimension score is(19.66 ± 4.31),the self-care responsibility dimension score is(16.38 ± 3.69),and the self-care skill dimension score is(29.09 ± 7.69)points,health knowledge level dimension score is(43.86 ± 14.04)points.Differences in the overall self-care ability of patients with different marital status,age,education level,place of residence and comorbidities have statistical significance.3 The average post-PCI patient health literacy score is(92.31 ± 19.65),the information accessibility dimension score is(37.37 ± 9.07),the financial support willingness dimension score is(7.39 ± 2.05),and the communication and interaction ability dimension score is(30.79 ± 7.20)points,the score of the willingness to improve health dimension is(16.22 ± 2.61)points.Differences in the overall health literacy level of patients with different marital status,age,education level and occupation type are statistically significant.4 Pearson correlation analysis results showed that the readiness for discharge after PCI was positively correlated with self-care ability(r = 0.569).In addition to the extremely low correlation between expected support and self-concept and health knowledge,the preparation for discharge The other dimensions of the degree are positively correlated with all dimensions of self-care ability(P <0.05);the post-PCI patient discharge readiness is positively correlated with health literacy(r = 0.501),except for the extremely low correlation between expected support and information acquisition ability In addition,other dimensions of hospital readiness are positively correlated with all dimensions of health literacy(P <0.05);self-care ability of patients after PCI is positively correlated with health literacy(r = 0.499),each dimension of self-care ability is consistent with all dimensions of health literacy It was positively correlated(P <0.05).5 Self-care ability plays a partial mediating role between health literacy and readiness to discharge.The intermediary effect of self-care ability between health literacy and discharge readiness is 53.09%.Conclusion1.Patients’ readiness for discharge after PCI is at a medium level,and there is a low level of readiness for hospital discharge.Marital status,age,education,monthly family income,place of residence,number of comorbidities,and depression are the main factors affecting patient readiness after PCI.2.The self-care agency of patients after PCI was at a medium level.Marital status,age,education,place of residence,and type of comorbidities are the main factors affecting the self-care agency of patients after PCI.3.Patients’ health literacy was low after PCI.Marital status,age,education,and occupation are the main factors affecting the health literacy of patients after PCI.4.Patients’ readiness for discharge after PCI was positively correlated with self-care agency and health literacy.The stronger the patient’s self-care agency,the higher the readiness for discharge;the higher the patient’s health literacy level,the higher the readiness for discharge.Nurses can improve patients’ readiness to discharge by improving their self-care agency and health literacy.5.The self-care ability of patients after PCI plays an intermediary role between health literacy and readiness to discharge. |
