Objective:To explore the intervention effect of emotion adjustment of traditional Chinese medicine on anxiety,depression,pain level and quality of life of patients with mild to moderate cancer pain,so as to provide theoretical basis for making appropriate and effective psychological intervention strategies for clinical nursing.Methods:Under the guidance of traditional Chinese medicine emotion theory,combined with literature review,the intervention program of traditional Chinese medicine emotion adjustment was initially formulated.After the demonstration of expert meeting method,the program was revised according to expert opinions,and combined with the problems existing in the pre experiment,the feedback of patients and their families,the program was further improved,and finally the intervention program of traditional Chinese medicine emotion adjustment was formed.Using the research design of randomized controlled trial,using the random number generated by computer,80 cancer pain patients with anxiety and depression were randomly divided into intervention group and control group,there were 40 cases in each group.The control group received routine nursing care of cancer pain,it mainly includes pain assessment,accurate administration,psychological nursing,health education and follow-up.The intervention group,on the basis of the control group,carried out the emotional adjustment program of traditional Chinese medicine,which was divided into two stages: centralized training and independent training.Concentrated training stage:the intervention content includes 5 courses,and the patients only receive one course for each hospitalization chemotherapy.Each course corresponds to different topics,each time lasting 1.5-2hours,and the intervention is completed before the start of the sixth chemotherapy cycle.Self training stage: after the second course,eight segments of brocade exercises and five elements of music therapy were carried out in daily life during hospitalization and during discharge(chemotherapy interval).Before and after the intervention,self-assessment anxiety scale,self-assessment depression scale,simplified McGill Pain Questionnaire and core quality of life scale were used to evaluate the feasibility and effectiveness of the intervention program.SPSS 21.0 statistical software was used to analyze the data.The statistical description adopts case number and composition ratio,mean plus minus standard deviation,median and interquartile spacing;statistical inference adopts t-test of two independent samples,paired t-test,nonparametric test,chi square test and Fisher precision test,with P<0.05 as the difference.Results:1.Comparison of general information between the two groups of patients: There were no significant differences in demographic characteristics between the two groups of patients,such as age,gender,education,marital status,and pain at admission(P>0.05),and they were balanced and comparable.2.Comparison of the psychological status of the two groups of patients: The SAS and SDS scores of the two groups of patients before the intervention were higher than the critical values of mild anxiety or depression,and the difference was not statistically significant(P>0.05);after the intervention,the scores of SAS and SDS in the intervention group were significantly lower than those in the control group,with statistical significance(P<0.05).The SAS and SDS scores of patients in the control group before and after the intervention showed no statistical difference(P>0.05).The SAS and SDS scores of the patients in the intervention group were lower than those before the intervention after receiving the traditional Chinese medicine emotion adjustment intervention(P<0.05).3.Comparison of pain levels between the two groups of patients: The total score of SF-MPQ and the scores of each dimension in the two groups before the intervention were balanced and comparable at baseline(P>0.05);after the intervention,the total score of SF-MPQ and the scores of each dimension in the intervention group were significantly lower than those in the control group(P<0.05).The pain level in the control group decreased after the intervention,but there was no statistical difference in the total score of SF-MPQ and the scores in all dimensions before and after the intervention(P>0.05).The total score of SF-MPQ and scores of each dimension after intervention in the intervention group were lower than those before intervention,and the differences were statistically significant(P<0.05).4.Comparison of the quality of life of the two groups of patients: The total score of EORTCQLQ-C30 and the scores of each dimension in the two groups before the intervention were balanced at the baseline level(P>0.05);the comparison between the two groups after the intervention showed that in addition to the dimensions of diarrhea and economic burden,there were statistically significant differences in total quality of life scores and scores in other dimensions(P<0.05).In the control group,after receiving conventional cancer pain care,the score in the symptom area of loss of appetite decreased,and the difference was statistically significant(P<0.05),there were no significant differences in the scores in the remaining 14 areas before and after the intervention(P>0.05);after the intervention group received emotion adjustment of traditional Chinese medicine intervention,the physical function,fatigue,and loss of appetite in EORTCQLQ-C30 were better than those before the intervention(P<0.05),only the score of economic burden had no significant change before and after the intervention(P>0.05).Conclusion:The emotion adjustment of traditional Chinese medicine implemented in this study can reduce the anxiety and depression level of patients with mild to moderate cancer pain,avoid the patients from falling into the negative ruminant state of fear of adverse drugreactions and aggravation of the disease,and help the patients to effectively control the pain,reduce the pain feeling and degree,and improve their quality of life.As one of the methods of non drug therapy,emotion adjustment of traditional Chinese medicine adheres to the overall concept of traditional Chinese medicine nursing and the basic characteristics of dialectical nursing.It has distinct Chinese cultural characteristics,conforms to the emotional characteristics of Chinese people,and is simple and easy to use.It is worth popularizing and applying.
