Objective To explore the changes of peripheral blood neutrophil count, lymphocyte count and erythrocyte volume distribution width with the progress of hepatitis b;The relationship between the blood cell parameters of neutrophil/lymphocyte ratio(NLR)and erythrocyte volume distribution width(RDW)and hepatitis b and hepatitis b cirrhosis was also investigated.methods Choose our digestive department, infection, physical examination center and the hospital in the southern district of infection cases total of 162 people,50 cases of healthy control group,50 patients with hepatitis b,from 62 patients with liver cirrhosis after hepatitis b complicated with hepatitis patients, blood routine application of clinical laboratory automatic blood analyzer test directly obtained RDW,absolute terms according to the absolute value of neutrophils and lymphocytes NLR,mutual contrast and analyze its corresponding value change differences between 3 groups, and the other 62 patients with liver cirrhosis after hepatitis b hepatitis group Child-pugh score system for each grade of liver function,The correlation between NLR and RDW level and child-pugh score was analyzed.Results(1)the hepatitis b group, hepatitis b cirrhosis group and healthy control group of three groups of people were compared, found that the value of NLR gradually increased;(2)the same three groups of people are compared, found to have a higher level of RDW;(3)in patients with cirrhosis after hepatitis b,with the aggravation of liver inflammation damage,NLR level showed a significant upward trend, and RDW also showed an upward trend;(4)linear analysis showed that child-pugh score increased with the increase of NLR,so NLR level was positively correlated with child-pugh score of patients with hepatitis b cirrhosis(P<0.05),and RDW level was positively correlated with child-pugh score of patients with hepatitis b cirrhosis(P<0.05).ROC analysis showed that the area under the curve of NLR and RDW for the diagnosis of hepatitis b cirrhosis(LC)was 0.687 and 0.816 respectively, both of which were statistically significant. When the cut-off values of the two parameters were 2.25 and 14.25,the sensitivity was 0.581 and 0.726,and the specificity was 0.8 and 0.8,respectively.Conclusion The numerical values of NLR and RDW can be said to be closely related to the development of hepatitis B and hepatitis B cirrhosis. The development of chronic hepatitis into cirrhosis, the process of further development of cirrhosis into decompensation period will lead to the respective levels of NLR and RDW.Therefore, the combination of NLR and RDW with child-pugh scoring system can more effectively assist the prediction of liver function in patients with hepatitis b,assess the degree of liver fibrosis, and determine by the area under the ROC curve(AUC).Compared with NLR,the accuracy of RDW is still higher.
