ObjectiveTo explore the relationship between serum bilirubin level and carotid—femoral pulse wave velocity in patients with essential hypertension,and to provide a reference for the clinical evaluation of arteriosclerosis by using serum total bilirubin level.MethodsA retrospective study was conducted on 797 hypertensive patients and non-hypertensive patients who were admitted to the General Hospital of the First Affiliated Hospital of Fujian Medical University from January 2016 to March 2018.Basic information such as age,medical history,and smoking history of all subjects was recorded.The height and body mass of all subjects were measured and the body mass index was calculated.Blood glucose,blood lipids,renal function and liver function were measured by fasting venous blood in the morning.The peripheral blood pressure and heart rate were measured by OMRON HEM-7052 electronic sphygmomanometer.Complior Analyse automatic arteriosclerosis and cardiac function detection system measures carotid-femoral pulse wave velocity(cf PWV).All subjects were divided into hypertension group and non-hypertension group,and then stratified analysis by gender,age,and diabetes.Total bilirubin was analyzed by stratification of the total bilirubin median 12.2 μ mol/L in the hypertension group.The basic information comparison between the two groups was performed by independent sample t test,and the comparison between groups was analyzed by one-way ANOVA.Correlation analysis and multiple linear stepwise regression analysis were used to analyze the influencing factors of cf PWV.ResultsThe levels of cf PWV and total bilirubin in patients with hypertension were significantly higher than those with normal blood pressure(P<0.05).The total bilirubin level of males after gender grouping was higher than that of females(P<0.05).Males in the normotensive group had higher cf PWV than women(P < 0.05).According to theage group,the young and middle-aged group(<65 years old)and the old group(≥65years old)showed that the cf PWV of the elderly group was significantly higher than that of the young and middle-aged population(P< 0.05),and there was no statistically significant difference in total bilirubin(P>0.05).The stratification of diabetes patients showed that cf PWV was higher in patients with hypertension than in patients with hypertension(P<0.05).Total bilirubin was lower than that of patients with simple hypertension(P<0.05).In the hypertensive group,the median total bilirubin was 12.2μmol/L,and the total bilirubin was <12.2 μmol/L and total bilirubin was ≥12.2 μmol/L.The cf PWV of the total bilirubin <12.2 μmol/L group was higher than the total bilirubin ≥12.2 μmol/L group(P<0.05).Correlation analysis showed that age,body mass index,systolic blood pressure,diastolic blood pressure,heart rate,fasting blood glucose,urea nitrogen,and creatinine were positively correlated with cf PWV in the general population(r=0.466,0.130,0.537,0.221,0.102,0.104,0.239,0.260,P<0.05);total bilirubin,high-density lipoprotein cholesterol,and glomerular filtration rate were negatively correlated with cf PWV(r=-0.104,-0.114,-0.278,P<0.05).Age,body mass index,systolic blood pressure,diastolic blood pressure,creatinine and urea were positively correlated with cf PWV in hypertensive patients(r=0.307,0.176,0.390,0.304,0.145,0.158,P<0.05);total bilirubin,Glomerular filtration was negatively correlated with cf PWV(r=-0.100,-0.249,P<0.05).Systolic blood pressure,diastolic blood pressure,creatinine,fasting blood glucose,low-density lipoprotein cholesterol,and heart rate were positively correlated with cf PWV in normal blood pressure(r=0.442,0.416,0.315,0.201,0.174,0.151,0.160,P<0.05);High-density lipoprotein cholesterol and glomerular filtration were negatively correlated with cf PWV(r=-0.122,-0.184,P<0.05).Multiple linear regression stepwise analysis showed that age,systolic blood pressure,body mass index,and total bilirubin were the influencing factors of cf PWV in the hypertensive population;in normal blood pressure population: age,systolic blood pressure,and heart rate were the influencing factors of cf PWV.Age,systolic blood pressure,and total bilirubin are the influencing factors of cf PWV in male hypertensive patients.Age,body mass index,systolic blood pressure,and diastolic blood pressure are the influencing factors of cf PWV in female hypertensive patients.Conclusion1.In the hypertensive population,especially in male hypertensive patients,total bilirubin levels,age,and systolic blood pressure the influencing factors of cf PWV.2.Among the people with normal blood pressure,age,systolic blood pressure and heart rate are the main influencing factors of cf PWV.
