Chronic heart failure(CHF)is a kind of clinical chronic medical syndrome which appears in the late stage of various serious cardiovascular diseases.Many patients diagnosed with heart failure are accompanied by mild depression and anxiety symptoms.The suffering of the disease will also make them lose confidence in life.Because of the particularity of heart failure patients,they not only bring great trouble to themselves and their families,but also cause serious economic burden to the society to a certain extent.Heart failure groups need to cause widespread concern of the community.The author found that the medical staff of the Second Affiliated Hospital of Nanchang University can only take care of the external physiological diseases of heart failure patients due to the limited time and energy,and seldom pay attention to the internal psychological problems of patients.The modern medical model of our country emphasizes the concept of "body and mind" health,and the medical social workers can provide psychological support and emotional guidance for patients with heart failure,which coincides with the professional value orientation of social work.The purpose of this study is to improve the self-efficacy of patients with heart failure and help them to build up their confidence in overcoming the disease.The research method is mainly based on the perspective of "body mind" whole person health mode,using professional methods such as case study and group work,trying to implement services from the aspects of physical health,psychological problems and spirit,and accordingly putting forward the role cluster of medical social workers at different levels In the actual process,many obstacles are encountered and effective countermeasures are put forward.In this study,the patients in cardiovascular medicine department of the Second Affiliated Hospital of Nanchang University were selected as the research objects,using meditation practice,breathing practice,music therapy and other skills,combined with group activities such as greeting card writing and chorus to provide services for the patients.Although good responses have been achieved at a certain level,there are also many difficulties in the practice process,which can be roughly divided into: Firstly,Body level.Secondly,the psychological level.Including the doubt of the service object to the professional nature of medical social work and the direct purpose of the group members;thirdly,the spiritual level.The mobility of patients and the "selective" service of medical social workers.The nature of the above problems not only involves hospitals,patients,medical social workers and other subjects,but also is objectively reflected in the whole practice process and service effect.In order to solve the above conflicts,the author puts forward some suggestions,such as shortening the group activity cycle,training the group "opinion leaders",introducing professional social work institutions,opening and tolerance + appropriate publicity.The highlights of this study are as follows: Firstly,The selection of research objects is representative;Secondly,The whole body and mind health model is the main thread throughout the whole paper;thirdly,The roles that can be played by different levels of body and mind are put forward intuitively in combination with practice;Fourthly,The individual case and group work are carried out simultaneously to provide all-round services for patients with heart failure;Fifthly,The activities are divided into body,mind and spirit Carry out special services.
