Study On Medical Records Adopted Treatment Proceeded From Kidney Of Ye

Objective:By sorting out related medical records concerning kidney treatment adopted in Guide to Clinical Practice with Medical Records,the etiology and pathogenesis,therapeutic principles and methods,characteristics of kidney disease,and prescription medication rules from various related medical records in respect of kidney treatment adopted in Guide to Clinical Practice with Medical Records are explored in this paper.Based on the theory of warm disease differentiation and modern research findings on treatment,academic characteristics,characteristics of disease,and prescription medication rules of Ye’s adopted treatment based on kidney are investigated in this paper.The study further digs academic connotations of related medical records concerning Ye’s adopted treatment based on kidney,enriches the connotation of the “Kidney Viscera-state Doctrine”,which can effectively guide clinical prescription medication and improve the clinical efficacy.Methods:By collecting and sorting out medical cases related to kidney treatment in Tian-Shi Ye’s Guide to Clinical Practice with Medical Records,annotations have been made through medical records on etiology,pathogenesis,disease names,syndrome names,therapeutic principles,therapeutic methods,prescriptions,traditional Chinese medicine,acupuncture,constitution,nursing and care,etc.Classification,quantification and mathematical statistics have been conducted on these above-mentioned,and Tables of Word data and data statistics have been established for related medical records concerning Ye’s adopted treatment based on kidney.Results:This study involved a total of 127 medical records adopted treatment based on kidney of Ye,including 115 cases of internal diseases,9 cases of gynecological diseases,1 case of surgical disease,1 case of pediatric disease and 1 case of ENT disease.The etiological statistical results showed that the six evils and five pathogens were the most affected,followed by bothersome,pathological products of phlegm and retained fluid.According to the statistical results,the pathogenesis of medical records can be divided into 13 categories:kidney yin deficiency,kidney yang deficiency,kidney qi deficiency,kidney essence deficiency,kidney meridian,kidney exogenous syndrome,and relationship disorders of spleen-kidney,liver-kidney,heart-kidney,stomach-kidney,kidney-bladder,kidney-multiple organs,lung-kidney.Syndrome name and acupuncture statistics are not of practical significance.The content of care includes five categories: diet,emotion,daily life,drug,sexual intercourse.The medical records treatment contain a total of 154 herbs with the cumulative usage times of 1136.The top 10 frequently used traditional Chinese medicine were: Poria Cocos(80),prepared Rhizome of Rehmannia(67),Ginseng(60),Yam(40),Licorice(35),Alisma Orientale(35),Cornus Officinalis(34),Schisandra Chinensis(32),Dwarf Lilyturf Tuber(31),Donkey Hide Gelatin(29),Cortex Moutan(29);The cumulative category frequency proportion of traditional Chinese medicine was demonstrated ranked in the top four: tonifying herbs,(510,44.9%),herbs for removing dampness by promotingdiuresis(135,11.9%),herbs for clearing heat(102,9.0%),astringent herbs(101,8.9%),;Temperature-induced drugs had the largest cumulative frequency of traditional Chinese medicine,and accounted for the largest proportion of total drugs.Sweet drugs was most commonly used among the five flavours,similarly,channel tropism drugs ranked first among those classified into the heart channel of Hand-Shaoyin.A total of 39 treatment methods were counted in medical records,and the top five were tonifying the spleen and kidney,warming the spleen and kidney,nourishing the liver and kidney,nourishing the kidney essence,and nourishing the kidney and nourishing qi.Fumai Decoction,Liuwei Pills,Shenqi Pills,Huqian Pills,Zishen Pills,and Zhenwu Decoction were used more frequently in medical records adopted treatment based on Ye’s adopted treatment based on kidney.Conclusions:1.In the medical records of adopted kidney treatment proceeded by Ye Tianshi,Syndrome of Qi,blood and fluid in internal medicine and gynecology prenatal and postpartum disease syndrome accounted for the most,which reveals that Ye gives priority on qi,blood and body fluid in the treatment based on kidney.2.In the medical records of adopted kidney treatment proceeded by Ye Tianshi,the six exogenous pathogenic factors have been regarded as the primary etiology of the medical records adopted treatment based on kidney,with apoplexy and fire evil for the most,the phlegm as the second etiology,and the etiology of sexual intercourse not reflected;the pathogenesis of kidney based viscera is mainly kidney yin deficiency,and the pathogenesis of other organs based on viscera is mainly spleen kidney imbalance.3.It is common a syndrome without a name in the medical records of adopted kidney treatment proceeded by Ye Tianshi with a few cases of acupuncture and moxibustion collected in the medical records adopted treatment based on kidney of Ye,and it is unlikely to accurately sort out its clinical application rules and characteristics.The nursing methods and contents introduced in medical records adopted treatment based on kidney of Ye are of great universality.However,significant differences are not presented in the medical records,which is hard to highlight the characteristics of the “Kidney Viscera-state Doctrine” as the basic nursing and care.Secondly,the argument on nursing and care contents is not deep enough and the scope is limited;4.Four kinds of traditional Chinese medicine are used most frequently in medical records of adopted kidney treatment proceeded by Ye Tianshi including: tonifying herbs,herbs for removing dampness by promoting diuresis,herbs for clearing heat,and astringent herbs;Majority of the selected traditional Chinese medicine have the property of warm,neutral and chill.Herbs with warm property are the priority while both warm and chill are combined.The flavor compatible characteristics of the selected Chinese medicine comply with the therapeutic principle of Shaoyin disease in the “Inner Canon” saying "the successful treatment of Shaoyin disease lies focusing herbs with sweet flavor on tonifying,combining herbs with bitter and pungent flavor on strengthening,supplementing herbs with sour and salty flavor".Medication focuses on adjusting and supplementing the five internal organs,especially on strengthening kidney by tonifying heart and spleen;5.Therapeutic methods in medical records of adopted kidney treatment proceeded by Ye Tianshi can be divided into two categories: the tonifying and supplementing method takes the dominant and the warming through method ranks the second.The tonifying and supplementing method can be subdivided into the dominant method of nourishing Yin and afew cases of invigorating qi and blood.The warming through method is composed of three kinds: the Taiyang warming and dispersing method,the Taiyin warming and tonifying method and the Shaoyin warming yang method.6.As for the prescriptions applied in the medical records of adopted kidney treatment proceeded by Ye Tianshi,both classical prescriptions and contemporary prescriptions were adopted,focusing on uniform reinforcing and avoiding powerfully tonifying.Luiwei Pill,Huqian Pill and Zishen Pill were commonly used for tonifying kidney yin and clearing heat;Shenqi Pill and Zhenwu Decoction were the first selected prescription for warming and invigorating kidney yang;Fumai Decoction and Guipi Decoction were used as the main prescriptions to balance nutrition and nourish blood.The application of prescriptions displayed three characteristics: tonifying kidney yin and clearing heat,warming kidney yang and activating kidney qi,and balancing nutrients and nourishing blood.
