Because of the characteristics of emergency services,emergency departments have an irreplaceable role in the medical service system of today's hospitals.In practice,due to the unreasonable configuration of medical staff in the emergency system,some nodes in the emergency system have blockage of the consultation queue,the service intensity is very high,and the other nodes are idle.Based on this,,the emergency queuing network system is constructed to reflect the flow of patients when they are in treatment,and find out the problems existing in the current emergency department medical staff allocation based on the analysis of specific treatment process of the current hierarchical triage mode.Meanwhile,the medical staff allocation model is built from two aspects of optimizing the performance indicators of the queuing system and reducing the labor cost index of medical staff in order tooptimize the existing allocation scheme,and provide theoretical basis for the actual allocation decision of medical staff.The research contents of this thesis mainly include:(1)This thesis fully investigates the research status and characteristics of China's emergency department,and analyzes its treatment model.Through the relevant theoretical concepts of the queuing network system and the actual status of the medical treatment process,the emergency queuing network system is constructed,including the definition of each service node within the network system,the analysis of the patient's transfer flow at each service node,and the basic queuing system construction of each service node and so on.(2)The queuing network model in the emergency department is constructed,and is composed of two parts.One part is the arrival distribution model of each node of the patient,including the patient's internal distribution and the external arrival distribution of each node;the other part is the establishment of the queueing model for each node in the system,including the M/M/C queueing model based on hypothesis and the improved M/G/C queueing model based on this.(3)This thesis starts from optimizing the performance index of queuing network system and reducing the labor cost index of medical staff and rearranges the medical staff in the system.Fuzzy decision model and multi-objective planning model are combined to construct a fuzzy multi-objective medical staff allocation model due to the non-quantity of the selected index.Based on the relevant ideas of linear programming,the PULP tool is used to solve the model.(4)In the specific case analysis,it is found that the emergency department has problems such as queuing congestion caused by the unreasonable configuration of medical personnel through the queuing network model constructed in this thesis.The medical staff in each node in the case is reallocated after applying the fuzzy multi-objective medical staff allocation model.According to the comparison and analysis before and after optimization,it is found that the indexes of the whole queuing system and each node have changed positively.For the whole system,the overall length of waiting queue and waiting time of patients in the queuing network system have decreased significantly,which shows that after the optimization of the medical staff configuration,the blocking situation of the queuing network system has been improved;for individuals,some nodes with relatively saturated indicators before optimization can be alleviated and improved greatly after optimization.The results of the case prove that the medical staff allocation model based on the queuing network system proposed in this thesis is reasonable and effective.The purpose of this thesis is to optimize the configuration of medical staff in the current emergency department through the proposed medical staff allocation model based on the queuing network system,and solve the problems caused by the unreasonable medical staff configuration,such as queueing obstruction,low medical service rate and high load intensity of medical staff.This is of great significance to the development of emergency departments in China.
