Objective: To investigate and analyze the clinical data of Clostridium difficile infection in hospitalized patients with diarrhea,summarize and summarize the relevant treatment of adult Clostridium difficile infection patients in our hospital,and explore the clinical characteristics and risk factors of Clostridium difficile infection patients.Methods: A retrospective investigation was conducted on the diarrhea patients in the second hospital of Hebei Medical University from January 2015 to January 2019,and the clinical data of the patients diagnosed as C.difficile infection were collected.According to the combination of C.difficile infection,it was divided into infection group(CDI group)and control group(non CDI group).Result:1.Study included 230 cases,107 cases in the CDI group and 123 cases in the non-CDI group;55 cases(51.4%)of men and 52 cases(48.6%)of women in the CDI group age of 50(11,87)years;74 in non-CDI(62.2%)and 49 in women(39.8%),with a median age of 49(16,86)years.2.Compared with the non CDI group,there was no significant difference in fever,fecal blood,intestinal obstruction,leukocyte count,ESR,high-sensitivity C-reactive protein,serum creatinine,albumin and hemoglobin reduction in the CDI group.Under the endoscopy(graduation)of CDI patients: mild,moderate and severe,pseudomembrane was its endoscopic characteristic performance;clinical characteristics were based on symptoms(diarrhea frequency,character,fever,abdominal pain)According to the analysis,88 cases(82.2%)were light patients,10 cases(9.4%)were medium patients,9 cases(8.4%)were severe patients,104 cases(97%)were improved by clinical drug treatment,the improvement rate was high,the curative effect was significant,9 cases were severe patients,6 cases were improved and 3 cases were not Two patients were discharged automatically,one was an old woman(82 years old).There were many basic diseases such as coronary heart disease,atrial fibrillation,heart failure,pneumonia,hypertension and so on,and finally died of sepsis.3.And non CDI groups P diseases.CDI group was more likely to suffer from gastrointestinal diseases,Cardiovascular diseases,nervous system diseases and blood/immune system diseases.(P<0.05),while other underlying diseases were not significantly associated with CDI disease status(P 0.05).4.Multivariate analysis used a logistic stepwise regression model.The analysis showed that compared with the non-CDI group,the CDI group had a history of surgery(OR = 6.734;95% CI 2.692-15.849;P = 0.000)and the number of comorbidities(OR = 3.215;95 %CI 1.576-4.743;P = 0.003),gastrointestinal diseases(OR = 4.135;95% CI 3.048-11.416;P = 0.000),antibiotic use(OR = 5.996;95% CI 2.173-15.481;P = 0.000)And the use of nitroimidazoles(OR=4.769;95%CI 2.138-14.757;P=0.000)is a risk factor for patients with CDI(P<0.05).Conclusions:1.The number of mild to moderate patients is mostly clinical,and most patients have significant therapeutic effects.2.Antibiotic use,increased number of comorbidities,history of surgery,and gastrointestinal diseases are risk factors for adult CDI.3.The clinical manifestations and laboratory examinations of CDI patients have no specific manifestations,and the pseudomembrane is a characteristic manifestation under endoscopic.
