Objective:The Study aimed to explore the correlation between serum levels of ANGPTL8 and intracranial arterial atherosclerotic stenosis in patients with cerebral infarction,achieve a better understanding of the relationship between ANGPTL8 and Cerebral infarction subjects with intracranial arterial atherosclerotic stenosis status.Method:Participants were enrolled between September 2018 and October 2019 at the Department of neurology in our Hospital.A total of 293 subjects were recruited.Among them,223 subjects with cerebral infarction complicated with intracranial artery atherosclerotic stenosis were the case group and 140 subjects with no cerebral infarction but complicated with intracranial artery atherosclerotic stenosis were selected as the control group.All participants were accepted MRI、DWI and cervicocerebral CT angiography.Record the information of each subject,including Age、gender、body mass index,intracranial arterial stenosis status,hypertension,family history of cerebrovascular,diabetes mellitus,Current smoking,Habitual alcohol drinking,and concentrations of lipid parameters,such as,triglyceride(TG),total cholesterol(TC),high-density and low-density lipoprotein cholesterol(LDL-C,HDL-C),glycosylated hemoglobin A1c(Hb A1c),homocysteine(Hcy),Alanine aminotransferase(AST),Aspartate aminotransferase(ALT),uric acid(UA),and the ANGPTL8 levels.In according with the Samuel,S standards,the subjects were divided into four groups: normality(30%),mild stenosis(30-49%),moderate stenosis(50-69%),severe stenosis(70-100%).The differences of anthropometric and biochemical characteristics between the case group and the matched group and the group with intracranial arterial stenosis in patients with cerebral infarction were compared respectively.The association of ANGPTL8 level with the degree of intracranial artery stenosis in patients with cerebral infarction was determined with spearman correlation coefficients.The ROC curve to revealed that the best cut off value for circulating ANGPTL8 to predict the intracranial arterial stenosis(≥30%),Ordered multivariate regression analyses was performed to identify independent relationship with intracranial arterial atherosclerotic stenosis.Result:1.Compared with the case group,there were no significant relationship were observed between the matched group in age,gender,BMI,intracranial arterial stenosis status,hypertension,family history of cerebrovascular,diabetes mellitus,Current smoking,Habitual alcohol drinking and ANGPTL8(P>0.05).It was proved that cerebral infarction itself had no significant effect on serum ANGPTL8 level.2.There were without statistically differences compare intracranial arterial stenosis in patients with cerebral infarction to intracranial arterial stenosis with no cerebral infarction in age,gender,BMI,family history of cerebrovascular,diabetes mellitus,hypertension,Current smoking,Habitual alcohol drinking(P>0.05),there is comparability between the groups.Compared with the patients with cerebral infarction had no intracranial arterial stenosis,the level of Hcy、ANGPTL8 were higher than in the Patients with cerebral infarction had Mild intracranial arterial stenosis(P<0.05);the level of TC,Hcy,ANGPTL8 were higher than in the patients with cerebral infarction had Moderate intracranial arterial stenosis(P<0.05);the level of TC,Hcy,Hb A1 c,ANGPTL8 were higher than in the patients with cerebral infarction had severe intracranial arterial stenosis(P<0.05).Compared with the patients with cerebral infarction had Mild intracranial arterial stenosis,the level of ANGPTL8 were higher than in the patients with cerebral infarction had Moderate intracranial arterial stenosis(P<0.05);the level of ANGPTL8 were higher than in the patients with cerebral infarction had severe intracranial arterial stenosis(P<0.05).Compared with the patients with cerebral infarction had Moderate intracranial arterial stenosis,the level of Hb A1 c 、 Hcy 、ANGPTL8 were higher than in the patients with cerebral infarction had severe intracranial arterial stenosis(P<0.05).3.According to Spearman correlation analysis,ANGPTL8(rs=0.80,P<0.001)、Hb A1c(rs=0.60,P<0.001)levels were positively correlated with Cerebral infarction subjects with intracranial arterial atherosclerotic stenosis status.TG(rs =0.43,P<0.001)、LDL-C(rs =0.15,P=0.02)、TC(rs =0.54,P<0.001)、Hb A1c(rs =0.52,P < 0.001)were positively correlated with ANGPTL8 level.4.Logistic regression results showed that ANGPTL8(OR=1.006,95%CI:1.004~1.008,P<0.001)、TG(OR=1.477,95%CI:1.008~2.005,P=0.012)and Hb A1c(OR=2.852,95%CI:1.789~5.547,P<0.001)were dependent risk variable was applied to examine the intracranial arterial atherosclerotic stenosis.5.The ROC curve to analysis revealed that the best cut off value for circulating ANGPTL8 to predict the intracranial arterial atherosclerotic stenosis was 1012.1pg/ml(sensitivity 98.2%,specificity 84.3 %,Diameter 0.24,and AUC: 0.90,95%CI 0.95-0.99,P=0.000).Conclusion:Our study suggests that ANGPTL8 levels in Cerebral infarction subjects are significantly and independently associated with risk of intracranial arterial atherosclerotic stenosis degree.Therefore,we were draw inspiration that ANGPTL8 might be serve as a novel biomarker for Cerebral infarction with intracranial artery Atherosclerotic stenosis.
