ObjectivesTo observe the intracranial atherosclerotic plaque vulnerability by high-resolution MRI,and analyze the related risk factors in the patients with acute cerebral infarction.Further study on the risk factors of culprit plaques can provide data for the identification of intracranial atherosclerotic plaques and a reference for the prevention and treatment of acute cerebral infarction.MethodsRetrospective study was conducted on the patients who had received high-resolution MRI diagnosed with acute cerebral infarction in our hospital from November 1,2016 to October 31,2020.The records included gender,age,smoking and alcohol consumption history,blood lipids,hypertension,diabetes,coronary heart disease,uric acid level,blood homocysteine and other general data.Patients were divided into vulnerable plaque group and non-vulnerable plaque group according to whether vulnerable plaques were detected.The vulnerable plaque group was divided into culprit plaque group and non-culprit plaque group according to whether culprit plaques were detected.SPSS 24.0 system was used to analyze the risk factors and logistic regression analysis was performed.P <0.05 was statistically significant.Results1.The history of hypertension,moderate to severe homocysteinemia and moderate to severe luminal stenosis were more common in the vulnerable plaque group than those in the control group,and the differences were statistically significant.2.Hypertension(OR 2.812,P =0.002)and moderate to severe arterial lumen stenosis(OR3.569,P = 0.000)were independent risk factors for plaque vulnerability.3.The only significant difference between the culprit plaques group and the non-culprit plaques group was the degree of moderate and severe stenosis,while the remaining data showed no difference between two groups.According to the analysis of risk factors,moderate and severe luminal stenosis(OR1.983,P =0.043)was found to be the risk factor for culprit plaques.There was no statistical significance in the rest.Conclusion1.Hypertension and moderate to severe stenosis of arterial lumen are independent risk factors for intracranial plaque vulnerability in acute cerebral infarction.2.Moderate and severe intracranial artery stenosis is a risk factor for culprit plaque formation in patients with acute cerebral infarction.
