Clinical Observation Of Yiyuan Jiangtang Decoction Based On The Therapy Of Healthy Qi-enriching And Mass-removing In The Treatment Of Impaired Fasting Glucose(Qi Deficiency And Phlegm Dampness Type)

Objective:To analyze the distribution of TCM Constitution between the impaired fasting glucose and diabetes,and provide the theoretical basis for the prevention of diabetes through the constitution management;through the observation of the clinical effect of Yiyuan Jiangtang Decoction based on the therapy of healthy qi-enriching and mass-removing on the patients with impaired fasting glucose(Qi deficiency and phlegm dampness type),in order to supply a new idea for the treatment and the delay and block of the occurrence and debelopment of diabetesMethods:1、Select the patients with impaired fasting glucose and the patients with type 2 diabetes in the database of the First Affiliated Hospital of Heilongjiang University of traditional Chinese medicine in 2019,and investigate and compare their distribution of TCM Constitution2、From October 2018 to December 2019,the patients with impaired fasting glucose(Qi deficiency and phlegm dampness type)were collected from the disease prevention center of the First Affiliated Hospital of Heilongjiang University of traditional Chinese medicine.Conduct the non randomized simultaneous control trial,the patients were divided into treatment group and control group according to the treatment intention of the patients.The control group was treated with basic intervention.The treatment group was treated with traditional Chinese medicine on the basis.The course of treatment was 8 weeks.After 8 weeks,the fasting blood glucose,TCM syndrome score and the change of TCM Constitution conversion points of the two groups were collected and comparedResults:1、Comparing the distribution of TCM constitution between 140 patients with impaired fasting glucose and 156 patients with type 2 diabetes mellitus,the first three biased constitution of the two groups were Qi deficiency,phlegm dampness and damp heat,while the ratio of yin deficiency constitution in diabetic patients was significantly higher than that of patients with impaired fasting glucose(P<0.05)2、A total of 56 patients with impaired fasting glucose were collected for clinical observation,including 27 patients in the treatment group and 29 patients in the control group.In the data of this study,the frequency of biased constitution reached 87.5%.The highest frequency of biased constitution was Qi deficiency in 28 cases(50.0%),followed by phlegm dampness in 26 cases(46.4%).After comparison,the intervention measures in the two groups had a certain effect on the reduction of fasting blood glucose(P<0.01),and the treatment group was better than the control group(P<0.01).In the control group,there were 3 cases(10.3%)with obvious effect,20 cases(69.0%)with effective effect,6 cases(20.7%)with no effect,and the total effective rate was 79.3%.In the treatment group,there were 11 cases(40.7%)with obvious effect,12 cases(44.4%)with effective effect,and 4 cases(14.8%)with ineffective effect,and the total effective rate was 85.2%.In the aspect of improving TCM syndrome score,TCM syndrome score in the treatment group decreased significantly(P<0.01),the total effective rate in the treatment group was 92.6%,and the total effective rate in the control group was 13.8%,the difference was statistically significant(P<0.01).In the aspect of improving TCM Constitution,the conversion scores of damp heat and qi stagnation in the control group decreased(P<0.05);in the treatment group,except Yin deficiency,the other were improved(P<0.05).After intervention,the conversion points of qi deficiency,phlegm dampness and damp heat in the treatment group were lower than those in the control group,with statistical significance(P<0.05)Conclusion:1、The most common biased constitution of impaired fasting glucose and diabetes are Qi deficiency,phlegm dampness and damp heat.Yin deficiency may be the key constitution for the transformation of impaired fasting glucose to diabetes2、The Yiyuan Jiangtang Decoction,based on the therapy of healthy qi-enriching and mass-removing,can obviously alleviate the impaired fasting glucose(Qi deficiency and phlegm dampness type),reduce the fasting plasma glucose value,regulate the biased constitution,improve the general symptoms,delay or block the diab etes.
