Objective:In this study,FRAX tools and bone mineral density(BMD)were analyzed and compared in the applicability and fracture prediction ability of hip osteoporotic fracture of elderly women in Yueyang area,and the fracture intervention thresholds of the two tools were preliminarily discussed,so as to provide theoretical basis for the prevention and treatment of osteoporotic fracture in Yueyang area.Methods:1.A total of 244 female patients aged 60-89 years who were hospitalized in our hospital from January 2016 to August 2019 due to osteoporotic hip fracture were collected as the fracture group.All patients underwent BMD examination of the femoral neck on the opposite side of the fracture.2.A total of 202 elderly women aged 60-89 who did not have hip fracture in the physical examination center and outpatient department of our hospital in the same period were collected as the control group.All patients underwent BMD examination of the left femoral neck.3.Take the moment before fracture as the time point,FRAX tool was used to calculate the probability of hip fracture within 10 years in both groups with and without BMD.4.ROC curve was used to compare the accuracy of BMD and FRAX tools in predicting the risk of hip fracture.At the same time,Youden index was calculated to get the best intervention threshold.Results:1.The differences of age,height,weight and the number of people with previous fracture history between the fracture group and the control group were statistically significant(P < 0.05).There was no significant difference between the two groups in the history of hip fracture of parents,smoking,corticosteroids,rheumatoid arthritis,secondary osteoporosis and alcohol consumption(P > 0.05).2.The BMD of femoral neck in fracture group was lower than that in control group(P < 0.05).3.Under the condition of combining BMD and not combining BMD,the probability of hip fracture calculated by FRAX tool in fracture group within 10 years was higher than that in control group,the difference was statistically significant(P < 0.05).4.The area under ROC curve of hip fracture was 0.615,0.595 and0.633 with or without FRAX tool and BMD alone,which indicated that the above prediction methods had certain value,and BMD alone had the highest prediction value(P < 0.05).5.The best intervention thresholds of FRAX tool combined with BMD or not and BMD only were 3.75,3.55 and-2.45,respectively.Conclusion:1.FRAX tool and BMD have certain value in predicting hip fracture of old women in Yueyang area.2.BMD alone is better than FRAX in predicting hip fracture in elderly women in Yueyang area.3.In Yueyang area,the best intervention thresholds of FRAX tool combined with BMD or not and BMD only were 3.75%,3.55 % and-2.45,respectively.
