Clinical Observation Of Syndrome Differentiation Based On Control Of Proteinuria In The Treatment Of CKD Stage 3b-5 Patients

Objective: The purpose of this study is to observe the progress of chronic kidney disease(CKD)stage 3b-5 patients with proteinuria treated by traditional Chinese medicine(TCM),explore related factors of CKD progress,analyze changes in experimental data and changes in clinical symptoms,and objectively and scientifically evaluate the clinical efficacy and safety of traditional Chinese medicine.To provide effective research data for the treatment of chronic renal failure(CRF)with traditional Chinese medicine.Methods:Selection experiments into standard,30 patients with CKD3b-5 period by comparing before and after,foundation treatment,and according to its medicinal broth of traditional Chinese medicine symptoms taking,the total course of 8 weeks.Various indicators of the patients before and after treatment were recorded,including serum creatinine(Scr),24-hour urinary protein quantification,urea nitrogen(BUN),cystatin C(cys-c),and TCM syndrome score.The glomerular filtration rate was calculated by e GFR[ml/(min·1.73m2)]=175×(Scr,mg/dl)-1.234×(年龄)-0.179×(0.79女性).Spss17.0 software was used to analyze statistical significance based on statistical knowledge.Results:1.Statistical analysis of observation indicators: 2.52 ± 0.64 g before 24-hour quantitative treatment of urinary protein,1.64 ± 0.83 g after treatment,P < 0.01,with significant statistical significance;Serum creatinine288.69 ± 68.02umol/L before treatment and 260.61 ± 64.42umol/L after treatment,P < 0.01,with statistically significant difference.Blood urea nitrogen treatment before 15.10 ± 4.93mmol/L,after treatment 13.52 ±4.47mmol/L,P < 0.01,there was a significant statistical significance.Serum cystatin level was 3.03 ± 0.71mg/L before treatment and 2.70 ± 0.68mg/L after treatment,p < 0.01,which was statistically significant.The glomerular filtration rate was 18.51 ± 5.49ml/min before treatment and 21.48 ±7.35ml/min after treatment,p < 0.01,which was statistically significant.1case of 3b stage before CKD staging treatment,21 cases of 4 stage,8 cases of5 stage,4 cases of 3b stage after treatment,19 cases of 4 stage,7 cases of 5stage,p > 0.05,no statistical significance.2.Statistical analysis of clinical efficacy: The overall effective rate was determined to be 93% according to the disease efficacy of urinary protein;According to the disease curative effect of creatinine,the overall effective rate was 87%.The overall effective rate was90%.3.Safety evaluation: No obvious side effects occurred during the treatment.Conclusion:1.Symptomatic treatment with Traditional Chinese medicine can effectively reduce ck D3B-5 urinary protein;2.Syndrome differentiation of proteinuria can reduce serum creatinine,urea nitrogen,serum cystatin;3.Treatment of urinary protein can increase glomerular filtration rate;4.Symptomatic treatment with TCM can improve the clinical symptoms of patients with CKD3B-5.
