ObjectiveThrough an effective questionnaire survey for elderly patients with Type 2 Diabetes Mellitus(T2DM),we can understand the status quo of sleep quality, negative emotion and quality of life in elderly patients with T2DM;analyze the impact of general demographic characteristics on sleep quality, negative emotion and quality of life in elderly patients with T2DM;explore the relationship among sleep quality, negative emotion and quality of life in elderly patients with T2DM;further analyze the factors affecting the quality of life in elderly T2DM patients, provide reference basis for improving the sleep quality, reducing the negative emotion and improving the quality of life in elderly T2DM patients, and provide reference theoretical basis for clinical nursing and intervention of T2DM patients.MethodsThis study was conducted by a cross-sectional survey method during the period from September 2018 to August 2019,230 elderly patients with T2DM were selected from a tertiary general hospital in Kaifeng City for questionnaire survey. The investigative tools included general demographic data questionnaire,PSQI(Pittsburgh sleep quality index),HADS(Hospital Anxiety and Depression Scale),DSQL(Diabetes Specific Quality of Life Scale).According to the criteria of inclusion and exclusion,a total of 230 questionnaires were sent out in this study,221 of which were valid. The effective rate of recovery was 96.09%..Epidata3.1 was used to establish the database,spss21.0 was used to analysis statistical, using percentage, using frequency and mean ± standard deviation to descriptive statistics of general data of elderly patients with T2DM;using independent sample t-test and one-way ANOVA to compare the differences of sleep quality, negative emotion and quality of life in elderly patients witn T2DM under different conditions;using the pearson correlation test to analyze the correlations among sleep quality, negative emotions and quality of life in elderly patients with T2DM;using multiple linear regression analysis of the influencing factors of the quality of life in elderly patients with T2DM..Results(1)The average score of sleep quality of 221 elderly patients with T2DM was(6.99 ± 1.69)points,and the incidence of sleep disorders was 29.9%.There were significant differences among gender, age, marital status, education level, residence, family per capita monthly income, smoking, drinking, exercise,BMI,diabetes duration in elderly patients with T2DM.(2)The average score of anxiety of 221 elderly patients with T2DM was(6.31 ± 1.90)points,and the incidence of anxiety was 23.1%;the average score of depression was(6.77 ± 3.41)points,and the incidence of depression was 30.8%.There are significant differences in anxiety scores of patients among gender, age, marital status, education level, residence, family per capita monthly income, medical insurance type, drinking;there are significant differences in depression scores among age, marital status, education level, residence, medical insurance type, smoking, drinking, exercise in elderly patients with of 221 elderly patients with T2DM.(3)The average score of QOL of 221 elderly patients with type 2 diabetes was(81.08 ± 18.32)points,and the highest score of psychological / mental dimension was(3.24 ± 0.49)points.There were significant differences among gender, age, education level, residence, family per capita monthly income, medical insurance type, smoking, drinking,BMI,course of diabetes, treatment, comorbidity in elderly patients with of 221 elderly patients with T2DM.(4)There was a positive correlation between sleep quality scores and quality of life scores in elderly patients with of 221 elderly patients with T2DM(r = 0.690,P < 0.05);anxiety scores was positively correlated with total quality of life scores(r = 0.701,P<0.05);depression scores was positively correlated with quality of life scores(r = 0.685,P<0.05);sleep quality scores was positively correlated with anxiety and depression scores, respectively(r = 0.676,P < 0.05;r = 0.658,P< 0.05).(5)The total score of quality of life as the dependent variable and the independent variable as the single factor analysis(P< 0.05),the multivariate stepwise regression analysis was performed. The results showed that: complications, treatment methods, type of medical insurance,BMI,Negative emotions(anxiety, depression)and sleep quality entered the final equation, explaining 53.7% of the total variance of quality of life in elderly patients with of 221 elderly patients with T2DM.Conclusion(1)The elderly patients with T2DM have poor sleep quality and high incidence of negative emotions.High scores of patients' quality of life indicating that their quality of life is poor, and psychological / dimensional is the most affected aspect.(2)There are significant correlations among sleep quality, negative emotion and quality of life in elderly patients with T2DM.The worse sleep quality, the worse negative emotions, and the worse quality of life.(3)Complications,treatment,medical insurance type,BMI,negative emotions(anxiety, depression)and sleep quality are the main factors affecting the quality of life in elderly patients with T2DM.
