Research BackgroundStandardized training for residents is a rigid requirement for entering clinical work.All physicians who intend to engage in clinical work after graduation must receive standardized training and can only take up their posts after passing the qualification.For physicians personally,the training is the initial stage of professional development.The doctors who have successfully passed the training have greatly improved their professional skills and personal qualities.For the medical industry,the standardized training system has alleviated the shortage of medical resources in China to a certain extent.The trainees still face obstacles in the doctor-patient relationship.First of all,the trainees are relatively young and lack sufficient mental preparation.They are more likely to have psychological problems than senior doctors.Second,trainees are likely to cause doctor-patient conflicts due to lack of skills and knowledge to communicate with patients.At present,the government has issued relevant medical and health laws calling on the whole society to respect medical personnel.Paying close attention to the doctor-patient relationship of trainees is a practical demand of the national society and individual doctors.Knowing the subjective feelings and needs of the trainees can build a better relationship between doctors and patients,alleviate the contradiction between doctors and patients,and improve the professionalism of trainees.PurposesThe purpose of this study is to explore the doctor-patient relationship of the trainees in Shandong Province.The specific goals are as follows:(1)to investigate the current status of the doctor-patient relationship in Shandong Province;(2)Exploring the reasons that affect the doctor-patient relationship of trainees;(3)Put forward reasonable solutions and suggestions for personnel training.MethodsThis study uses a combination of qualitative and quantitative methods.Using the DDPRQ-10 "difficult" doctor-patient relationship scale,an electronic questionnaire survey was conducted on trainees in 9 large comprehensive top three hospitals in Shandong Province.Taking the department as a unit,the group sampling method is adopted to select the training students from 9 departments including general practice,obstetrics and gynecology,and pediatrics.After eliminating invalid questionnaires,a total of 3528 valid questionnaires were recovered,with an effective rate of 78.40%.Perform descriptive statistics on the data to obtain the status of training the doctor-patient relationship of the trainees,and perform single factor analysis and regression analysis to obtain the factors that affect the doctor-patient relationship.Based on the results of the quantitative analysis,the in-depth interviews with the trainees of a hospital in Jinan in the study were conducted to inquire about the doctor-patient relationship in the department,the trainees ’personal experiences,needs,and their views on the new media platform.After interviewing 29 training students,the information reached saturation.The situation analysis method was used to analyze the interview information,and the matrix checklist was used to summarize the interview results to supplement and explain the results of the quantitative research.The software used for data analysis is SPSS 24.0 and Nvivo 11.0.ResultIn this study,the average age of the included samples was 29.38±2.60 years old.The average score of DDPRQ-10 is 30.00±4.26 points.The relationship between doctors and patients in internal medicine and pediatrics is tense,and the relationship between doctors and patients in orthopedics is the best.51.8%of the training students had a poor doctor-patient relationship.Women and the department where they are orthopedics are positive factors for the doctor-patient relationship;managing more than 7 beds is a negative factor for the doctor-patient relationship.Qualitative interviews consist of three themes,which are to train students’subjective feelings about the current status and causes of the doctor-patient relationship,required training and other needs,and their views on the new media platform.The interview contents of the training students are organized into a matrix checklist,which can be divided into three dimensions:the training students think they are related to themselves,the training students think they are related to the patient,and the training students think they are related to other aspects.The current status and causes of the doctor-patient relationship found 9 influencing factors:workload,communication,job responsibilities,legal awareness,health education,economics,psychological reasons,news coverage and trust.Among them,workload and communication are the most influential factors mentioned.Trainees have their own training needs and expectations of patients and society.Most of the trainees believe that the new media platform is not useful,but hope that the media can guide patients to the correct information.Conclusions and policy recommendationsThis study analyzes the current status and reasons of the doctor-patient relationship among Shandong standardized trainee.The main conclusions are as follows:(1)In this study,Shandong standardized trainee’ relationship between doctors and patients is relatively tense,and the proportion of poor doctor-patient relationships is 50%.(2)Gender,department,and number of management beds are the influencing factors of the doctor-patient relationship,and workload and communication are the main factors affecting the doctor-patient relationship.The relationship between doctors and patients is better for females and disciplined standardized trainer whose department is orthopedics.The relationship between doctors and patients with excessive workload and lack of communication skills is even worse.(3)In order to improve the current tension between doctors and patients,the standardized trainee not only have their own needs,but also the needs of patients and society.The trainees hope that they can receive more training on communication skills and medical insurance knowledge,so that their rights and interests can be protected,and hope that patients can consciously maintain the medical order,and society and the media can shape the correct values.Based on the results of this study,the following suggestions are proposed:(1)Strengthen communication and training to prevent doctor-patient disputes;(2)Actively popularize health knowledge and promote correct values;(3)Accelerate the promotion of graded diagnosis and treatment system to ease the work of doctors in large hospital load.(4)Psychological counseling and medical ethics education are provided to regular trainees who have a poor doctor-patient relationship in internal medicine,general medicine,and pediatrics.
