| Background Renal anemia is one of the commen complications of hemodialysis.Erythropoietin(EPO)and iron supplement are the conventional treatment methods for hemodialysis complicated with renal anemia.Red blood cells(RBC),Hemoglobin(HGB),Mean Corpuscular Volume(MCV),Mean Corpuscular Hemoglobin(MCH),Mean Corpuscular Hemoglobin Concentration(MCHC)and serum iron(SI)are the most commonly used index for evaluating renal anemia.In the treatment process,these index can accurately judge the anemia degree of patients during hemodialysis and play a key guiding role in the treatment.Howener,in the clinical application of grugs to treat renal anemia,many patients develop refractory anemia,which has a serious impact on the life span and quality of life of hemodialysis patients.Therefore,early detection of the potential pathogenic factors of refractory anemia in the above patients is crual.The early onest of hemodialysis patients has been in the state of microinflammation,that is,the patients has no clear signs of infection and there is a continouous release of inflammation mediators in the body,resulting in a continouous inflammatory response in the body.Serum levels of inflammatory mediators C-reactive protein(CRP)and interleukin(IL-6),procalcitonin(PCT)in hemodialysis patients were increased to different degree.The above indicators were used to effectively evaluate the status of micro-inflammation and renal anemia in hemodialysis patients and analyze the correlation.Objective Anemia index of RBC,HGB,MCV,MCH and MCHC,SI content in maintenance hemodialysis patients and serum levels of inflammatory mediators CRP,IL-6 and PCT were uesd to investigate the correlation between inflammatory state and renal anemia in hemodialysis patients.Methods Restrospective analysis was performed on 40 patients with maintenance hemodialysis patients admitted to nanyang central hospital from January 2018 to January 2019.According to C-Reactive Protein(CRP)levels were divided into a microinflammatory group(CRP 5~8mg/L)with 23 patients and a non-microinflammatory group(CRP <5mg/L)with 17 patients,and 20 healthy patients were selected as the control group.Maintenance hemodialysis patients was detected in RBC,HGB,MCV,MCH,MCHC,SI and IL-6,the PCT levels and healthy check up RBC,HGB,MCV,MCH,MCHC,SI and IL-6,the PCT levels.Results1.Among the 40 patients with maintenance hemodialysis,there were 11 males and 12 females in the microinflammatory group,with an average age of(46.7±3.4)years.In the non-microinflammatory group,there were 9 males and 8 females,with an average age of(47.2±5.4)years.In the healthy group,there were 10 males and 10 females(46.2±4.1)years.The three groups were comparable in age and gender.2.Comparison of serum leveles of IL-6 and PCT in each group Serum IL-6 and PCT in the microinflammatory group were higher than those in the non-microinflammatory group and the control Serum IL-6 and PCT in the non-microinflammatory3.Comparison of leveles of RBC,HGB,MCV,MCH and MCHC,SI in each group The levels of RBC,HGB,MCV,MCH and MCHC,SI in the microinflammatory group were lower than that in the non-microinflammatory group and the control group.The levels of RBC,HGB,MCV,MCH and MCHC,SI in the non-microinflammatory group were lower than those of the control group.4.The corralation between serum inflammatory mediators and Anemia indicators in maintenance hemodialysis patients.According to the results of correlation analysis of maintenance hemodialysis patients’ serum IL-6,PCT levels were lnegatively correlated with RBC,HGB,MCV,MCH and MCHC,SI levels,the differences werestatistically significant(P<0.05).Conclusion1.In patients with maintenance hemodialysis,there is a state of micro-inflammation,which is related to renal anemia.2.Inflammatory mediators may cause refractory anemia and iron deficiency in hemodialysis patients by affecting the erythropoiesis and iron metabolism of the body. |
