Research backgroundHereditary Spherocytosis (HS) is a hereditary hemolytic disease with red cell membrane deficiency. HS is the most common hereditary hemolytic disease in northern Europe and North America. It was reported that the morbidity of HS in these area was1/5000in1961, and it raised up to1/2000in1990s. HS is common in Japan too and accounted for74.2%in congenital hemolytic disease in Japan according to the data punished by hemolytic disease committee of Japan. Although there was not crowd investigating data of HS at present, there were cases presentation around China. According to statistic of hemolytic disease constituent ratio in some hospital, HS accounts for1.2%in Guangzhou hospital,2.7%at Harbin Medical University,8.8%at China Medical University. It was reported by LiJinying that there were112cases of HS amid356cases of hemolytic diseases, which had been diagnosed in The Changhai affiliated hospital of Second Military Medical University. The proportion of HS was31.5%, including12cases of HS suffer from Mediterranean anemia at the same time. So HS is not a seldom disease.Research PurposeCompare and evaluate the diagnostic value of the there screening tests,mean spherical corpuscular volume and mean corpuscular volume, mean corpuscular hemoglobin concentration and flow cytometric osmotic fragility test.MethodCollect peripheral blood of the20patients who were diagnosed and treated as HS,35patients who were diagnosed as mediterranean anemia,50testes who were Physical examination qualified in the first affiliated hospital of GuangXi medical university. Examine them with the there methods, evaluate the diagnostic value of these methods by Statistical analysis their Sensitivity and specificity.ResultThe sensitivity is90%specificity is96.47%when use MSCV<MCV as the optimum cut-off point. While the sensitivity is only45%specificity is94.12%when use MCHC>355g/L as diagnostic criteria. The sensitivity becomes85%specificity becomes97.65%when use flow cytometric osmotic fragility test residual red cells%<23.6%as the optimum cut-off point.Conclusion1.when MSCV<MCV, the sensitivity and specificity of HS are high, the sensitivity is90%and the specificity is96.47%.2. when MCHC>355g/L, the HS sensitivity is low, which is45%, and the specificity is acceptable, which is94.12%.3.when the remaining red blood cells%of flow cytometric osmotic fragility test<23.6%, the sensitivity is85%and the specificity is97.65%.
