| Objective:Sarcopenia is a syndrome characterized by a decrease in the amount and function of skeletal muscle.In the past few years,with the increase of the elderly population,the detection rate has also increased,resulting in a serious decline in the quality of life of patients.The mechanism of chronic heart failure(CHF)complicated with sarcopenia is very complex,and the severity of cardiac function will affect the prevalence of sarcopenia.This paper mainly explored the correlation between chronic heart failure and sarcopenia,and explore the risk factors of sarcopenia in patients with chronic heart failureexplored the pathogenesis between the two,deepened the understanding of sarcopenia and formulated prevention and treatment measures,so as to provide corresponding research support for the prognosis and severity of CHF patients.Methods:From September 2018 to March 2020,109 patients who were inpatients in the Department of Geriatrics of the First Affiliated Hospital of Wannan Medical College,Yijishan Hospital,met the diagnostic criteria for chronic heart failure.109 patients were the heart failure group and 65 healthy controls were the control group.The height and weight of all selected subjects were collected,and the patients with heart failure group were classified by NYHA.All subjects underwent dual X-ray Absorptiometry(DXA)tests to compare the prevalence of sarcoma in the heart failure group and the normal control group;and then compared the detection rates of men and women in the two groups differences;logistic regression analysis of risk factors for sarcopenia.Then study whether there is a correlation between central function classification and the incidence of sarcopenia in the heart failure group.Finally,the study subjects of the heart failure group and the normal group were divided into(40.0-49.9 years)group,1(50.0-59.9years old)group 2,(60.0-69.9 years old)group 3,(70.0-79.9 years old)group 4,(80 years old or above)group 5 were compared for the detection rate of sarcopenia and the skeletal muscle index of the extremities of all ages.Statistical analysis was performed using t test,chi-square test and logistic regression analysis.Results:(1)The detection rate of sarcopenia in the heart failure group(32.1%)was higher than that in the normal group(13.8%);the detection rate of sarcopenia in the two groups over 80 years old was higher.(2)The difference of ASMI between heart failure group and normal group was statistically significant.Comparison between the two groups:the man with ASMI(kg/m~2),total muscle mass(kg),ALM(kg),BMI(kg/m~2),weight(kg),arm lean mass(kg)and leg lean mass(kg)in the normal group were all greater than those in the heart failure group;the gril with total muscle mass(kg),ALM(kg),height(m),lean mass(kg)and leg lean mass(kg)of the control group were also higher than those of the heart failure group.There was prominent by significant difference between the two terms.(3)The detection rate of sarcopenia(72.2%)was highest in the heart failure group in cardiac function level III.(4)Sarcopenia was highest in the heart failure group over the age of 80(60.0%),and in the normal group was highest in the sarcopenia group(50%);And the ASMI in the two groups over 80 years was lower than the 40-49.9years age group and 50-59.9 years age group.Conclusion:(1)The incidence of sarcopenia was higher in patients with CHF,and the incidence of sarcopenia was higher with the poorer cardiac function.(2)Chronic heart failure and ASMI are the risk factors of sarcopenia.The older the person and the less the muscle mass,the higher the incidence rate of sarcopenia.For the elderly patients with chronic heart failure,we should carry out the screening of sarcopenia as early as possible,find and intervene sarcopenia in time,so as to delay the progress of the disease. |
