Early diagnosis and early intervention of Alzheimer’s Demential(AD)to delay disease progression have been widely accepted.The research of AD in traditional Chinese medicine continues to deepen,but the dialectics of AD still relies on the traditional four diagnostic methods and lacks standardized examination ways.By analyzing the correlation between TCM syndromes and cognitive impairment,brain atrophy in early AD patients,this study hopes to help dialectical classify treatments of traditional chinese medicine.Methods: patients who were cured in hubei provincial hospital of traditional Chinese medicine from November 2018 to November 2019 were screened,and the patients were evaluated by professional doctors and researchers for TCM dialectics,cognitive function and cerebral atrophy.Finally,40 cases were included in cognitively normal groups(younger group has 20 cases,older group has 20 cases),71 cases were included in early AD groups,to evaluate the cognitive function and the degree of brain atrophy.Results:1.There was no significant difference in the baseline conditions between the younger AD patients and normal people.There was no significant difference between the older patients and normal people.2.In the younger groups,compared with the control group,the score of memory function with AD was decreased(P < 0.01),and the time of attention/executive function was increased(P < 0.05)in the marrow sea deficiency syndrome.The score of memory function decreased(P < 0.01)in the spleen-kidney deficiency syndrome.The score of memory,language were decreased(P < 0.01)and the time of attention/executive function was increased(P < 0.05)in the syndrome of deficiency of both qi and blood.3.In the younger group,the score of marrow sea deficiency syndrome were negatively correlated with memory function(r=-0.939,P < 0.01)and visual spatial structure ability score(r=-0.538,P < 0.05),and was positively correlated with attention/executive function time(r=0.509,P < 0.05).The score of the spleen-kidney deficiency syndrome was negatively correlated with the score of memory function(r=-0.502,P < 0.05).The syndrome of deficiency of both qi and blood’s score were negatively correlated with the score of memory function(r=-0.736,P < 0.01)and language function(r=-0.357,P < 0.05),was positively correlated with attention/executive function time(r=0.588,P < 0.05).4.In the younger groups,compared with the control group,the MTA score and Koedam score of patients were increased(P < 0.01,P < 0.05)in the marrow sea deficiency syndrome.The scores has no significant difference in MTA,frontal lobe index and Koedam between control group and the spleen-kidney deficiency syndrome.MTA score and Koedam score were increased in the syndrome of deficiency of both qi and blood(P < 0.05,P < 0.01).5.In the younger groups,TCM syndrome score of marrow sea deficiency syndrome were positively correlated with medial temporal lobe(r=0.868,P < 0.01)and parietal lobe atrophy(r=0.856,P < 0.01).The TCM syndrome score of the spleen-kidney deficiency syndrome was positively correlated with the atrophic grade of medial temporal lobe(r=0.706,P < 0.01).The syndrome of deficiency of both qi and blood’s score were positively correlated with the grades of medial temporal lobe(r=0.623,P < 0.05)and parietal lobe atrophy(r=0.674,P < 0.01)6.Compared with the elderly control group,memory function score of patients with AD medullary sea syndrome decreased(P < 0.01),attention/executive function time increased(P < 0.05),and visual spatial structure function score decreased(P < 0.05).7.Compared with the elderly control group,the improved MTA and Koedam scale score of AD medullary sea syndrome increased(P < 0.01,P < 0.05).Conclusions:1.There are differences between the syndromes of early AD in cognitive impairment and brain atrophy patterns.2.There was a significant correlation between the scores of various syndromes in early AD with the degree of cognitive impairment and brain atrophy.3.The differences in the cognitive impairment and brain atrophy patterns of the different TCM syndromes confirmed that the syndromes were different subtypes of early AD,rather than different periods in the disease progression.4.Compared with the younger AD patients of the marrow sea deficiency syndrome,the older patients showed a significant increase in the degree and type of cognitive impairment,and the degree of medial temporal lobe and parietal lobe atroph. |
