Objective:To analysis the cognitive function,Electroencephalogram and Hippocampal Changes in Mild Cognitive Impairment and Alzheimer's Disease's,explore the relationship among them.Method:Retrospectively analysis the 22 patients of MCI and 46 patients of AD visiting to the cognitive and memory disorder out-patient department of the first hospital of Jilin University during the July 2015 to December 2016.Record the clinical datas,Mini-Mental State Examination(MMSE),Memory and Executive Screening(MES),Montreal Cognitive Assessment(Mo CA),Activities of Daily Living(ADL),4h video-EEG results and MRI.According to the grading of Clinical Dementia Rating(CDR),the patients were divided into four groups:MCI(CDR=0.5),mild AD group(CDR=1),moderate AD group(CDR=2)and severe AD group(CDR=3).Use software SPSS17.0 to carry out the statistical analysis.Result:1.According to the CDR grade,dividethe patients into four groups:MCI groups,mild AD group,the moderate AD group and severe AD group.We find that there were no statistical significance in gender,age,level of education(P>0.05),while there were significantly statistical significance in the disease duration,MMSE score,Mo CA score,MES scores and ADL scores,electroencephalogram background rhythm,epileptic discharge and the MTA-scale scores(P<0.05);CDR score was positively correlated with MMSE score,Mo CA score,MES score and MTA-scale score,and negatively correlated with ADL score.2.According to the CDR score,we compared the EEG and MTA.We found that MCI patients with abnormal EEG rate reached 81.82%,significantly higher than that of the MTA(36.36%)and the abnormal rate of EEG in patients with AD was95.65%,slightly higher than that of the MTA(80.43%).3.Based on presence of epileptic discharge on EEG,the patients were divided into four groups:sample without epilepsy group,a small amount of epileptic discharge,a moderate munber of epileptic discharge and a large number of epileptic discharge.There were significantly statistical significance in MMSE score,Mo CA score,MES scores and ADL scores among the four groups(P<0.05),and the same result also seen in the total score of MTA-scale and the left side of the MTA-scale score(P<0.05).4.The MTA-scale score is possitively relative to the disease duration,namely with the extension of course,the MTA-scale score also increased,while there was nothing to do with the age and level of education;MTA-scale was positively correlated with MMSE score,MES score and Mo CA score,which means the high the MTA-scale sore was,the poor the cognitive function was,and the corrlation of the left MTA-scale was higher than the right.MTA-scale was negetively correlated with ADL score,namely the higher MTA-scale score was,the poor daily life ability the patients was,and and the corrlation of the left MTA-scale was higher than the right.Conclusion:1.As the course of the disease prolong,cognitive function drop accordingly.2.With dementia degree aggravating,the more obviously the cognitive function decline,the more severely the EEG change and the more severely the hippocampal atrophy was;3.With the amount of the epilepsy discharge increasing,the more obviously the cognitive function decline,the more severely the hippocampal atrophy was.4.The functional change of patients with cognitive impairment was earlier than that of the structure.
