Objective Using musculoskeletal ultrasound technology to quantitatively evaluate the clinical efficacy of Tai Chi Massage in the treatment of scapulohumeral periarthritis,to explore the theory and mechanism of Tai Chi Massage in the treatment of scapulohumeral periarthritis,to provide a basis for the clinical use of Tai Chi Massage treatment of scapulohumeral periarthritis.To explore the advantages and theoretical basis of musculoskeletal ultrasound in the diagnosis and efficacy evaluation of scapulohumeral periarthritis.Methods To recruit patients with scapulohumeral periarthritis diagnosed in the outpatient of Massage/Rehabilitation Medicine Department of the Chinese Medicine Hospital of Hubei Province from January 2019 to June 2019.The diagnosis of scapulohumeral periarthritis conforms to the Diagnostic Efficacy Standards for TCM Syndrome and the Clinical Diagnosis and Treatment Guide · Orthopedic Branch.Sixty patients with periarthritis of shoulder who met the diagnostic and inclusion criteria were randomly divided into massage treatment group(treatment group)and waiting treatment group(waiting group).In addition,30 healthy volunteers with age and gender matching the treatment group and the waiting group were recruited as the healthy group.Both the treatment group and the waiting group were treated with massage once a day for 20 minutes each time.After 5 consecutive treatments,patients could rest for 1 day in total of 10 treatments.The waiting group did not receive treatment during the trial period unless the trial ended.The treatment group and the waiting group were evaluated with the CMS score before and after treatment in this study.Musculoskeletal ultrasound was used to measure the thickness of Sub S,CAL,Supra S,Infraspinatus tendons and Teres minor tendons.Treatment group,waiting group and healthy group evaluated before the treatment.After treatment,only the treatment group and the waiting group were evaluated again.Results 1.Basic information analysis: this study included 30 healthy volunteers,including 14 males and 16 females.Fifty-five patients with scapulohumeral periarthritis were finally included.Among the 55 cases,there were 22 male patients(40%)and 33 female patients(60%).The incidence of left shoulder was 22(40%)and that of right shoulder was 33(60%).There were 30 cases of manual workers,accounting for 54.55%,and 25 cases of mental workers,accounting for 45.45%.The above information was statistically comparable,P>0.05.2.After treatment,the results of CMS score showed that compared with the waiting group,pain,daily life,range of motion,muscle strength and CMS total score in the treatment group all increased(P<0.05),and the above indexes in the treatment group also increased after treatment compared with those before(P<0.05).It is suggested that massage therapy can improve the pain,range of motion and daily life of patients with periarthritis of shoulder.3.Before treatment,the thickness of Sub S,CAL,Supra S,Infraspinatus tendons and Teres minor tendons all increased in the musculoskeletal ultrasound measurements in the treatment group and the waiting group compared with the healthy group(P<0.05).It indicates that musculoskeletal ultrasound has diagnostic significance in the measurement of ligament and tendon of scapulohumeral periarthritis.4.After treatment,the thickness of Sub S,CAL,Supra S,Infraspinatus tendons and Teres minor tendons were all reduced in the treatment group compared with the waiting group in the musculoskeletal ultrasonic measurements(P<0.05),and the above indexes were also reduced in the treatment group after treatment compared with those before(P<0.05).It shows that massage can improve the above indexes of periarthritis of shoulder,and musculoskeletal ultrasound can not only be used for the diagnosis of periarthritis of shoulder,but also has important significance in the evaluation of the therapeutic effect of periarthritis of shoulder.Conclusion 1.Based on the fact that scapulohumeral periarthritis belongs to the original deficiency standard,Taiji Massage using massage operation that can supplement the deficiency and increase the loss can effectively improve the pain,daily life,joint range of motion and muscle strength of scapulohumeral periarthritis patients.2.Combined with the results of musculoskeletal ultrasound before and after treatment,Tai Chi Massage can be targeted for treatment of Sub S,CAL,Supra S,Infraspinatus tendons and Teres minor tendons.3.Musculoskeletal ultrasound can be used not only for the diagnosis of periarthritis of shoulder,but also for the evaluation of the therapeutic effect of periarthritis of shoulder.
