ObjectiveTo investigate the changes of plasma renin activity and aldosterone levels in patients with primary hypertension.And the role of low-renin hypertension in primary hypertension.Detection of hypermethylation sites in the 11β-hydroxysteroid dehydrogenase 2(11β-HSD2)promoter in patients with low-renin hypertension,and comparison of the methylation rates between the low-renin hypertension group and the control group.It reveals the possible mechanism of the pathogenesis of low-renin hypertension.Methods150 untreated patients with primary hypertension in our hospital were admitted as the hypertension group and 50 healthy volunteers as the control group.Plasma renin activity and aldosterone levels were measured,and changes in plasma renin activity and aldosterone levels in the hypertension group and the normal control group were compared.Differences in plasma renin activity and aldosterone levels in patients with different blood pressure and age were detected.Hypertensive patients were grouped according to plasma renin activity levels and the composition ratio of low-rennin hypertension in hypertensive patients was analyzed.Detection of methylation of 11β-HSD2 promoter region(34-548)by Agena Mass ARRAY.47 hypertensive patients with low-rennin hypertension who were successfully detected by methylation were admitted as the low-rennin hypertension group and 20 healthy volunteers as the normal control group.The differences in methylation rates between the two groups were compared.ResultsThe plasma renin activity in the hypertension group was significantly lower than that in the normal control group,with a significant difference(P<0.05).Aldosterone’s difference is not statistically significant(P>0.05).With the increase of blood pressure,the plasma renin activity is gradually lower and the level of aldosterone is gradually higher.With the increase of the age,the plasma renin activity and aldosterone level gradually decrease,but the difference is not statistically significant(P>0.05).Low renin hypertension accounted for 31.3% of the hypertension group.The incidence of methylation in the 11β-HSD2 promoter region was significantly higher in low-renin hypertension group than in the normal control group(P<0.05).The methylation rate are higher at CpG11、12(129: 131bp);CpG15、16、17(159:161:166bp);CpG62、63、64(485:487:489bp);CpG65(502 bp);CpG72(542bp)in low-renin hypertension patients and the differences are significant(P<0.05).ConclusionsThe plasma renin activity in the hypertensive group was lower than that in the normal control group,aldosterone is higher than normal control group.With the increase of blood pressure,the plasma renin activity is gradually lower and the level of aldosterone is gradually higher.With the increase of the age,the plasma renin activity and aldosterone level gradually decrease.Low renin hypertension is an important subtype of essential hypertension.Methylation of 11β-HSD2 promoter region is involved in the pathogenesis of low-renin hypertension. |
