ObjectiveWith the increasing popularity of laparoscopic techniques in minimally invasive surgery,its application is gradually expanded and there is a trend towards to replace the traditional open abdominal surgery.At present,It preferred to establish pneumoperitoneum in laparoscopic surgery,and the most extensive use of the abdominal media is carbon dioxide(CO2).Both the compression of the CO2 on the internal organs and the absorption of the gas have a certain effect on the homeostasis of the patient in the intraoperative and postoperative conditions,which has not yet attracted the attention of the people.By com-paring the different pressure of carbon dioxide pneumoperitoneum on homeostasis of lapa-roscopic colorectal cancer patients,this paper will further explore the recovery of patients,and strengthen the management of patients in perioperative period to achieve the objective of rapid rehabilitationMethodsIn this paper,we will select 60 cases of patients with radical colorectal cancer from the Affiliated Hospital of Tai shan Medical University gastrointestinal surgery department as the observation object.And the patients with preoperative colonoscopy and pathological biopsy were confirmed as malignant tumors.According to the random digital table method,patients were divided into high pressure of CO2 pneumoperitoneum(high pressure group)and low-pressure of CO2 pneumoperitoneum(low pressure group),with 30 patients in each group.The pressure of CO2 pneumoperitoneum in low pressure group is selected to be 8 to 10mmHg and the high pressure group is 12 to 14mmHg.The patient’s gender,age,duration of CO2 pneumoperitoneum(min),surgical anesthesia time(min)and intraoperative blood loss(ml)were recorded.The PH,PaCO2,PaO2 were recorded and statistically calculated before surgery(T1),lh after the CO2 pneumoperitoneum(T2),2h after the CO2 pneumo-peritoneum(T3),lh after stop pneumoperitoneum(T4).The venous blood samples at T1-T3 and 24 hour(T5)after stopping pneumoperitoneum were collected for determination of serum concentrations of ALT,AST,Cr,BUNResults1.The general statistics:There was no statistically significant difference in patients with age(P>0.05).Although the patients with low pressure group had long period of CO2 pneumoperitoneum,had increased the time of operation anesthesia and much more intrao-perative blood loss,There was no statistically significant difference between the two group(P>0.05)2.The check indexes were compared in both groups at T1,there was no statistically significant difference3.Compared with T1,the PH of the two groups significantly decreased in the period of T2-T4.Compared with the low pressure group,the decrease in the high pressure group was more obvious(P<0.05),and the difference was statistically significant.Compared with T1,the PaCO2 of both groups were increased significantly during the time of T2-T4 Compared with the low pressure group,the high pressure group rise more obvious(P<0.05),and the difference was statistically significant4.Compared with the T1,the two groups in T2,T3 and T5 period the ALT,AST,BUN,Cr were significantly increased.Compared with the low pressure group,the decrease in the high pressure group was more obvious(P<0.05),and the difference was statistica?lly significant.ConclusionsUnder the different pressure of CO2 pneumoperitoneum,the laparoscopic colorectal cancer patients have different influence on the body homeostasis.The high pressure CO2 pneumoperitoneum group with more chance to change the acid-base balance and damage the liver and kidney function during the intraoperative and postoperative time more seri-ousIt is require that the surgical patients who will have laparoscopic radical colorectal cancer,should adopt the low CO2 pneumoperitoneum pressure,and reduce the change of the body homeostasis during the perioperative period,which is beneficial to the recovery of postoperative patients and the operation effect is more significant.
