| BACKGROUND: With the more and more cases that malignant cell was diffused in abdomen after laparosocpic (LP) surgery with carbon dioxide (CD) pneumoperitoneum(PNP) for malignant disease in clinic have reported, now there are some question about diagnostic LP or LP with CD PNP resection of malignant disease in abdomen .today there are some researches about it, The aim of the current study was to examine the influence of a PNP on the morphology of the peritoneum and the intraperitoneal tumor cells implantation and growth in the case of intrabdominal spread of tumor cells. And at the time will see Whether different gases used to establish PNP or laparotomy(LT) have an influence on tumor cell growth intrabdomen . MEATHEDS: thirty nude mice were stratified into three groups. every group has ten mice. Group 1 was laparotomy group, group 2 was LP group with CD PNP, group 3 was LP group with nitrogen PNP. After every rat was injected intraperitoneally 5×105 mammary adenocarcinoma cells (SHZ88 cell) ,thirty minutes later the operation begin . At the three groups select randomly 12 nude mice (every group has 4 nude mice) After operation 2 hour and 48 hour, open the nude mouse abdomen and examined its peritoneum by trans electron microscopy. Thirty days later kill the nude mice, open the abdomen and see the difference of intrabdominal tumor growth between the three groups. RESULTS: Under electron microscopy: No diffuse changes of the peritoneal surface and no diffuse metastases were observed in 1 group. In group 2,starting 2 hours after release of the PNP, strong alterations of the peritoneum become visible and the connection between mesothelial cell was broken and parts of the underlying basal lamina were laid bare .Tumor cells attached to the free basal lamina and formed predominantly diffuse metastases throughout the peritoneum within the next 48 hours. In group3, the alterations of the peritoneum issame as the group 2,but no tumor cells attached to the free basal lamina and formed predominantly diffuse metastases throughout the peritoneum within the next48 hours. The average weight of group 1 intraperitoneal tumor is 0.7368±0.18958 mg, the average weight of group 2 intraperitoneal tumor is1.7861±0.0399 mg, the average weight of group 3 intraperitoneal tumor is 0.8260±0.3559mg, The average weight of group2 intrabdominal tumor was significantly increased compared with group 3(p<0.01), The average weight of group3 intraperitoneal tumor compared with group 1 have no significantly difference (p>0.05). CONCLUSIONS: CD PNP promotes intrabdominal tumor cells growth and diffusion, Nitrogen PNP has no promotion. The reason that CD PNP is benefited to the tumor growth in abdomen is the CD but not the PNP. |
