| Objectives:Chronic kidney disease(CKD)is one of the diseases with the highest morbidity and mortality worldwide.Numerous studies have shown that inflammatory factors released by CKD play an important role in the progression of CKD disease and determine the prognosis of CKD.This study systematically and comprehensively analyzes the relationship between neutrophil-lymphocyte ratio(NLR),platelet-lymphocyte ratio(PLR),and monocyte-lymphocyte ratio(MLR)and clinical features of CKD,in order to explore NLR,PLR and MLR as the feasibility and effectiveness of assessing the inflammatory state and severity of CKD disease.Methods:Collecting a total of 112 patients with CKD in the Department of Nephrology of the First Affiliated Hospital of Kunming Medical University from January 2017 to June 2018,and 100 healthy subjects from the same period were selected as controls.Collect basic clinical data and laboratory indicators such as C-reactive protein,procalcitonin,blood routine,liver and kidney function,blood lipids,etc.calculate NLR,PLR and MLR,comparing the differences of NLR,PLR,MLR and the above clinical values in CKD and healthy controls;112 patients with CKD were divided into high NLR,high PLR,high MLR and low NLR,low PLR and low MLR groups according to the median NLR,PLR and MLR,comparing the clinical and laboratory indicators Differences;Finally,correlation analysis was used to analyze the correlation of NLR,PLR and MLR with laboratory indicators.Results:1.Compared with the healthy group,NLR,PLR and MLR were higher in the CKD group.the eGFR,lymphocytes,hemoglobin,albunin,lipoprotein,iron ions,calcium ions of CKD group were significantly lower than the control group(P<0.05),and platelet count,white blood cell count,C-reactive protein,procalcitonin,triglycerides3 ferritin,urea nitrogen,creatinine were higher than the control group(P<0.05).2.In the high NLR of CKD group,hemoglobin,total cholesterol,triglyceride,glomerular filtration rate,and hematocrit were lower than those in the low NLR group,and the differences were statistically significant.White cell count,neutrophil count,monocyte count,phosphorus ion,C-reactive protein,procalcitonin,urea nitrogen,creatinine,PLR and MLR were higher in the high NLR group than in the low NLR group(P<0.05).3.In the high PLR group,lymphocyte count and glomerular filtration rate were significantly lower than those in the low PLR group(P<0.05).Neutrophil count,phosphorus ion,C-reactive protein,procalcitonin,urea nitrogen,creatinine,NLR and MLR were higher than those in the low PLR group(P<0.05)4.In the high MLR group,lymphocyte count and glomerular filtration rate were lower than those in the low MLR group.Neutrophils,monocytes,C-reactive protein,procalcitonin,urea nitrogen and creatinine were lower in the high MLR group,and the difference was statistically significant(P<0.05).5.Spearman correlation analysis showed that(1)NLR is negatively correlated with hemoglobin,hematocrit,lymphocyte,glomerular filtration rate,total cholesterol,triglyceride(P<0.05).NLR is positive correlated with white cell count,neutrophil count,and platelet count,.monocyte eount,urea nitrogen,ereatinine,C-reactive protein,procalcitonin,ferritin,phosphorus ion,PLR and MLR(P<0.05).(2)PLR is negatively correlated with hemoglobin,hematocrit,lymphocyte count,glomerular filtration rate(P<0.05).PLR is positive correlated with white cell count,neutrophil count,Platelet count,urea nitrogen,creatinine,C-reactive protein,procalcitonin,Phosphorus ion,NLR,MLR(P<0.05).(3)MLR is negatively correlated with hemoglobin,hematocrit,lymphocyte count,glomerular filtration rate,total cholesterol,low-density lipoprotein(P<0.05).MLR is positive correlated with neutrophil count,urea nitrogen,creatinine,C-reactive protein,and Procalcitonin,phosphorus ions,and NLR,MLR(P<0.05).Conclusions:l.NLR,PLR and MLR are higher in CKD than those in healthy people.Patients with CKD exhibit a low-grade inflammatory state.2.In CKD,NLR,PLR and MLR were positively correlated with inflammatory markers such as CRP,PCT,Renal function index such as creatinine and urea nitrogen,but negatively correlated with eGFR.3.Due to its simplicity,speed and low price,NLR,PLR and MLR are expected to be effective indicators of disease severity and inflammatory status in patients with CKD,especially in primary hospitals. |
