Objective: To study the effect of orbicularis oculi muscle pre suture in blepharoplasty of middle-aged and old patients with obvious lower eyelid structural relaxation. After at least half a year to more than a year of visits and statistics,we compared and observed the time of incision healing, the recurrence rate of palpebral band, whether there were complications, and whether the patients were satisfied with the appearance. Methods: From July 2016 to July 2018,60 middle-aged and elderly patients were selected for eyelid pouch treatment in our department, with an average age of 45 to 65 years old,all of which met the following surgical conditions: 1.Obvious protrusion of orbital septum fat, accompanied by relaxation of lower eyelid skin and orbicularis oculi muscle.2.The wrinkles around the eyelid are obvious and accompanied by middle facial ptosis.3.Know the medical history and have no scar.4.For patients who are taking anticoagulant drugs, the operation can only be considered if the coagulation function is rechecked after the drug withdrawal for at least two weeks and the indicators are confirmed to be normal.5.Exclude poor heart function, patients with hypertension,etc.60 patients meeting the above conditions were randomly divided into two groups, namely the observation group and the control group,30 cases in each group, including 10 males and 20 females in the observation group. The traditional method was improved during the operation, and the orbicularis oculi muscle contraction was pre sutured for a short time;the control group was 13 males and 17 females, using the traditional method of blepharoplasty. The two groups of patients, gender and age were comparable. Results: By comparing the photos of 60 patients before and after operation, one year after operation, the patients’ satisfaction in the observation group was higher than that in the control group(P < 0.01).There was no significant difference in complications between the two groups(P > 0.05).The patients in the observation group had no hematoma or infection,no blepharoplasty relapse and obvious improvement ofperiocular wrinkles. Conclusion:For middle-aged and old patients with blepharoplasty,on the basis of traditional blepharoplasty, through the use of pre suture method to adjust the orbicularis muscle severed during the operation, after removing the excess slack part, the precise suture. After the contraction of orbicularis oculi muscle, the structure of lower eyelid was strengthened. The operation improved the blepharoplasty, but also played the role of lifting the middle face and annual lightening of the eyelid.Comparison of treatment satisfaction between the two groups: after one year’s follow-up of the two groups, the patients’ satisfaction of the observation group with shortened orbicularis muscle pre suture was significantly higher than that of the control group with traditional operation(P< 0.01);one year after the operation, there was no significant difference in the incidence of complications between the two groups(P> 0.05). |
