Objective:The levels of angiogeein-like protein 8(ANGPTL8),blood glucose,blood lipid,c-reactive protein and other indicators inpatients withcoronary heart disease were observed to evaluate its correlation With the incidence of coronary heart disease and the degree of coronary artery disease.Method:1.Patients admitted to the department of cardiovascular medicine of an affiliated hospital of Bengbu medical college fromOctober 2018 to June 2019 and who underwent coronary angiography were selected as the study subjects.Improve coronary angiography to confirmthe diagnosis of coronary heart disease(at least one coronary artery lesion degree≥ 500%),court the number of vascular lesions according to the angiography results,and conduct Gensini score to evaluate the severity of coronary artery lesions.According to the angiography results,165 patients in the coronary heart disease group and 78 patients in the non-coronary heart disease group were divided into the control group.2.General information and clinical data were collected,including age,gender,diabetes history,etc.and fasting blood samples were collected for biochemical analysis to understand the indicators of fasting blood glucose,lowdensity lipoprotein,triglyceride,c-reactive protein,etc.,and the plasma ANGPTL8 level of the subjects was detected by enzyme-linked immunosorbent assay(Elisa).3.Use SPSS23.0 statistical software for data analysis.The measurement data were expressed as mean ±standard deviation((?)±s).The counting data were compared between groups by chi-square test.Pearson or Spearman correlation analysis was used to analyze the correlation between the variables.The influencing factors of coronary heart disease were analyzed by binary Logistic regression analysis.The factors affecting the severity of coronary heart disease were analyzed by Logistic regression analysis.P<0.05 means the difference is statistically significant.Results1.Of the 243 patients,165 were in the CHD group and 78 in the control group.The results of one-way anova and chi-square test indicated that there were no statistically significant differences bet ween the CHD group and the control group in age,gender,TC,HDL,apoA,etc.The CHD group was significantly higher than the control group in terns of diabetes,FBG,TG,LDL,LDL-c,CRP,and plasma ANGPTL8 concentrations,with statistically significant differences.2.According to the results of coronary angiography,the subjects were divided into 50 patients with single-vessel disease,72 patients with double-vessel disease,and 43 patients with three-vessel disease.The results of one-way analysis of variance and chi-square test indicated that there were no significant differences at baseline in gender,age,TC,HDL,apoA and other factors among patients with coronary heart disease with different lesion couts(P>0.05).Patients with coronary heart disease with different lesion couts were significantly higher than the control group in terns of diabetes,FBG,TG,LDL,LDL-c,CRP,and plasma ANGPTL8 concentrations,with statistically significant differences(P<0.05).3.Pearson correlation analysis was conducted on fasting plasma ANGPTL8 concentration and age,FBG,TG and other indicators,respectively.The results showed that plasma ANGPTL8 level was positively correlated with FBG,TG,LDL,Idl-c,CRP,and Gensini scores(P<0.05).HDL was negatively correlated with plasma ANGPTL8(P=0.017,r=-0.153).However,the concentration of ANGPTL8 was not correlated with age,TC and apo A(P>0.05).4.Spearman correlation analysis was applied to court coronary artery pathological changes with age,whether suffering fromdiabetes,FGB,TG and other indicators were analyzed,and the resuts showthat the cout and whether suffering fromdiabetes,coronary artery lesions FBG,TC,TG,LDL,LDL-C,CRP and Gensini score were positively correlated(P<0.05),but in coronary artery lesion counts and were not associated with age,sex,HDL,apo A(P<0.05).5.The results of binary Logisitic regessi on analysis showed that only the plasma ANGPTL8 concentration was an independent factor affecting the incidence of coronary heart disease(P<0.01),and the other indicators had no significant correlation with the incidence of coronary heart disease(P>0.05).The results of the ROC curve indicated that the area under the ROC curve of the plasma ANGPTL8 concentration in patients with coronary heart disease was 0.854(0.807-0.901),the optimal cut-off value was 744.73pg/mL,the sensitivity was 72.10%,and the specificity was 83.3%.6.The results of the ordered Logsitic regession analysis showed that CRP and plasma ANGPTL8 concentration were independent factors affecting the number of coronary artery lesion count in patients with coronary heart disease(P<0.05),while other indicators had no significant correlation with the number of coronary artery lesion cout(P>0.05).Conclusion:1.The fasting plasma ANGPTL8 concentration was positively correlated with FBG,TG and LDL,and negatively correlated with HDL,whichfurther verified the important role of ANGPTL8 in glucose metabolismand lipid metabolism.2.Coronary heart disease grolp(especially three lesions)plasma ANGPTL8 level significantly increased compared with normal control group,were positively correlated with Gensini score,the concentration of CRP and plasma ANGPTL8 CHD patients with coronary artery lesion couts independent impact factors,prompt ANGPTL8 plasma concentration may be important factors predict the severity of coronary heart disease in patients with coronary artery disease,clinical go up can be used for the early detection and diagnosis of coronary heart disease,provides a newresearch direction of prevention and cure for CHD. |
