Objective:1.To observe the efficacy and safety of Juan-bi granule in treating wind-cold-dampness syndrome of KOA.2.To observe the effect of Juan-bi granule on serum inflammatory factors in patients with wind-cold-dampness syndrome of KOA.Objectively evaluate the anti-inflammatory effect and mechanism of KOA treated with Juan-bi Granule from cytokine level.Method:Sixty-six patients with wind-cold-dampness syndrome KOA who met the inclusion criteria were selected from the department of rheumatology of Yunnan hospital of traditional Chinese medicine.Random number table method was used for random grouping,treatment group and control group 33 cases.The treatment group was treated with Juan-bi granule.The control group was treated with diacerein capsule.Both groups were treated for 8 weeks.Serum inflammatory factors,laboratory efficacy indexes(ESR,CRP),clinical efficacy indexes(TCM syndrome score,WOMAC score and VAS score),and laboratory safety indexes were observed before and after treatment.These indicators were used to assess safety and efficacy.Result:1.Serum inflammatory factors(IL-1β,TNF-α,COX-2,IL-6):After 4 and 8 weeks of treatment,IL-1β,TNF-α,COX-2 and IL-6 in the two groups were compared with those before treatment with statistically significant differences(P<0.01).Before and after 8 weeks of treatment,there was no statistically significant difference in IL-1β,TNF-α,COX-2 andIL-6 between the two groups(P>0.05).After 4 weeks of treatment,there were statistically significant differences in IL-1β and TNF-α between the two groups(P<0.05),and between the two groups showed that the control group was superior to the treatment group,there was no significant difference in COX-2 and IL-6 between the two groups(P>0.05).2.Laboratory efficacy indicators(ESR,CRP):ESR and CRP in the two groups at 4 and8 weeks after treatment were statistically significant compared with those in the group before treatment(P<0.01).There was no statistically significant difference between the two groups in ESR and CRP before treatment,4 weeks after treatment,and 8 weeks after treatment(P>0.05).3.Clinical efficacy indicators(WOMAC score,TCM syndrome score,VAS score):The clinical efficacy indexes of 2 weeks,4 weeks,6 weeks and 8 weeks after treatment were statistically significant compared with those before treatment(P<0.01).Before treatment,2 weeks,4 weeks,6 weeks and 8 weeks after treatment,there was no statistically significant difference between the two groups in the clinical efficacy indicators(P>0.05).4.Efficacy evaluation index(disease efficacy and TCM syndrome efficacy):Both groups were effective after 2 weeks,4 weeks,6 weeks and 8 weeks of treatment.Comparison between the two groups in disease efficacy and TCM syndrome efficacy at 2,4,6 and 8 weeks,the difference was not statistically significant(P>0.05).5.Safety index: The safety indexes of the two groups after 4 and 8 weeks of treatment were compared with those before treatment,the difference was not statistically significant(P>0.05).Conclusion:1.Juan-bi granule can significantly improve the curative effect of diseases and TCM syndromes of patients,and effectively reduce ESR,CRP,VAS score,WOMAC score,etc.Its curative effect is the same as that of diacerein capsule,and it has a better therapeutic effect on KOA.2.Juan-bi granules may play an anti-inflammatory role by reducing serum IL-1β,TNF-α,COX-2,IL-6 and other inflammatory factors,so as to improve the condition ofKOA and protect the bone of knee joint.3.Juan-bi granule has no obvious adverse reactions in the treatment of KOA,and it is safe to some extent,which is worthy of clinical application and promotion.
