Objective:This study observes the curative effect of added Li Zhong Tang on patients with nonalcoholic fatty liver disease(NAFLD)and its effect on insulin resistance.On the basis of clinical observation,establish a NAFLD rat model,use added Li Zhong Tang to intervene,observe its effect on serum leptin and insulin resistance index of experimental rats,and explore the curative mechanism of Jiawei Lizhong Decoction on NAFLD.Methods:1.Clinical study:Sixty patients with NAFLD were randomly divided into treatment group and control group,30 cases in each group.Both groups were given general basic treatment.The treatment group was treated with added Li Zhong Tang,one dose per day,each dose was taken with 150 ml warm boiled water;the control group was treated with polyene phosphatidylcholine capsule,456 mg per time,three times a day,and the treatment course was three months.To observe the improvement of symptoms score,liver function(AST,ALT),blood lipid(TC,TG),FPG,FINS,IR,controlled attenuation parameter(CAP)determined by Fibro Touch and safety in the treatment of NAFLD.2.Experimrntal study:65 healthy SPF male SD rats were selected and 10 were randomly selected as the blank group.The remaining 55 rats were fed a high-fat diet(58% basic feed + 2% cholesterol + 40% lard)for 12 weeks to establish a NAFLD rat model.From the 8th week weekend to the 12 th week weekend,randomly select one rats observed the pathological changes of their liver until the model was successfully established.After the successful modeling,10 rats were randomly selected as the modle group.The remaining 40 rats in the experimental group were randomly divided into 4 groups,10 rats in each group,i.e.high dose group,medium dose group,low dose group and Western medicine control group.According to the table of conversion coefficient of body weight dose per kilogram of animal and human body,the dosage needed for the model experiment of rats was calculated.The high,medium and low dosage of added Li Zhong Tang modified suspension was given by gavage once a day for 4 weeks;the western medicine control group was given polyene phosphatidylcholine capsule by gavage once a day for 4 weeks at the dose of 142.5mg/kg/d;the model group was given equal volume distilled water 10 ml a kind of once a day for 4 weeks,the blank group was given 10 ml equal volume distilled water a kind of Kg-1,once a day for 4 weeks.At the end of the 16 th week,the liver morphology and pathological changes were observed,the body weight and liver index was calculated.Blood samples were collected to measure leptin,FINS,FPG and the insulin resistance index was calculated.Result:1.Clinical study:Compared with before treatment,after treatment,the scores of symptoms,liver function(AST,ALT),the blood lipids(TC,TG),FPG,FINS and IR of the two groups were significantly decreased,the difference was statistically significant(P<0.01).Compared with that before treatment,the CAP of liver in the two groups after treatment improved significantly(P<0.01).Compared with the control group,the improvement of symptom score,blood lipid(TC,TG),FPG,FINS,IR and CAP in the treatment group was better than that in the control group,and the difference was statistically significant(P < 0.01 or < 0.05).There was no significant difference in liver function(AST,ALT)between the treatment group and the control group(P >0.05).In the process of clinical research,there was no abnormal safety index,no side effects and adverse reactions.2.Experimental study: Body weight,liver wet weight,liver index: After successful modeling,compared with the blank group,the body weight,liver wet weight,and liver index of the model group increased(P <0.01);the high dosage group,the high dosage group and Compared with the model group,the weight,liver wet weight,and liver index decreased(P <0.01).Among them,the high dosage group had a better weight loss effect than the medium dosage group(P <0.05),and the high dosage group had a better liver wet weight and liver index.Medium dosage group(P<0.01);liver pathology: after successful modeling,compared with the blank group,the pathological section of the model group showed diffuse steatosis under the light microscope,visible fat cavities of different sizes,and some infiltration of inflammatory cells After treatment,compared with the model group,the liver pathological sections of the rats in each treatment group showed that the fat became better than the model group;serum leptin(LP),FPG,FINS,and HOMA-IR: after the model was successfully established,compared with the blank group In comparison,LP,FPG,FINS and HOMA-IR were all increased(P < 0.01);compared with the model group,LP,FPG,FINS and HOMA-IR of rats in the high dosage group and the medium dosage group were reduced.,(P <0.01);there was no significant decrease in the low dosage group of traditional Chinese medicine and the control group of western medicine(P>0.05);Compared with the Chinese medicine high dosage group,the high dosage group’s LP,FPG,FINS,and HOMA-IR reduction effects were better than the latter(P<0.01);the safety index was not see exception.Conclutions:Added Li Zhong Tang has a significant effect in the treatment of NAFLD.It can improve the symptoms score,liver function,blood lipid,FPG,FINS,HOMA-IR,CAP and liver imaging changes of patients,and has a strong safety and no adverse reactions.The experimental have shown that added Lizhong Tang can alleviate hepatocyte steatosis,fasting blood glucose,fasting insulin,insulin resistance index and serum leptin levels in model rats,improve insulin resistance and increase insulin sensitivity.
