Clinical Study Of Xinglou Chengqi Decoction In Treating Stroke-associated Pneumonia Of Heat-phlegm And Sthenic-fu Type

Objective: To explore the the clinical efficacy and safety of Xinglou Chengqi Decoction in treating Stroke-associated pneumonia(SAP)of Heat-phlegm and Sthenic-fu Type,so as to provide more clinical evidence for the treatment of SAP by integrated traditional Chinese medicine(TCM)and western medicine.Methods: A total of 60 patients who diagnosed with SAP of Heat-phlegm and Sthenic-fu Type admitted to the encephalology department of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from January 2019 to December 2019 and met the standard were selected.According to the random number table method,they were divided into treatment group and control group,with 30 cases in each group.The control group received conventional treatment according to the relevant western medicine guidelines,and the treatment group received oral or nasal feeding Xinglou Chengqi Decoction on the basis of the treatment in the control group for 10 days.Changes of TCM syndrome scores,the National Institutes of Health Stroke Scale(NIHSS),clinical pulmonary infection score(CPIS),White Blood Cell(WBC),C-reactive Protein(CRP)and Procalcitonin(PCT)were observed and recorded before and after treatment in the two groups,and the total clinical efficacy of the two groups after treatment was evaluated.During the treatment,the patients were monitored for safety indicators,such as Urine routine,sealing routine,liver and kidney function,and electrocardiogram,and the adverse reactions were recorded.SPSS 20.0software was used for statistical analysis of data to evaluate the efficacy and safety of Xinglou Chengqi Decoction in treating SAP of Heat-phlegm and Sthenic-fu Type.Result:1.Total clinical efficacy: the total clinical effective rate in the treatment group was 93.33%,and the total clinical effective rate of the control group was 80.00%.The total clinical effective rate of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).2.TCM syndrome score: after treatment,the TCM syndrome score of both groups was significantly lower than that before treatment(P<0.01).After treatment,the score of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).3.NIHSS score: after treatment,the NIHSS score in both groups was significantly lower than that before treatment(P<0.01).After treatment,the score of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).4.CPIS score: after treatment,the CPIS score in both groups was significantly lower than that before treatment(P<0.01).After treatment,the score of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).5.Inflammatory indicators WBC,CRP and PCT: after treatment,the levels of WBC,CRP and PCT in the two groups were significantly lower than before treatment(P<0.01).Moreover,after treatment,the reduced levels of WBC,CRP and PCT in the treatment group were better than those in the control group,and the difference was statistically significant(P<0.05).6.Safety evaluation: During the course of treatment,no abnormal changes were observed in the safety indexes of the two groups,such as erbianroutine,liver and kidney function and electrocardiogram.Mild adverse reactions were found in 1 patient in the treatment group and 2 patients in the control group,and there was no statistically significant difference between the two groups in the cases of adverse reactions(P>0.05).Therefore,the drug safety of both groups was judged as grade II.Conclusion: 1.Based on the TCM theory of the lung and the large intestine being interior-exteriorly related,adding Xinglou Chengqi Decoction to treat SAP of Heat-phlegm and Sthenic-fu Type to conventional western medicine treatment,can further improve the patient’s TCM syndrome performance of patients,restore their damaged nerve function,reduce the symptoms of pulmonary infection,reduce the levels of three inflammatory indicators,WBC,CRP and PCT,and improve the clinical efficacy.2.Xinglou Chengqi Decoction can not only improve the symptoms of lung infection in patients,but also have a good effect on the recovery of damaged nerve function.3.The drug safety of Xinglou Chengqi Decoction is good,which is worthy of clinical research and promotion.
