Study On The Correlation Between Youth-type Pouch And Qi-deficiency Constitution And The Surgical Treatment Of Orbital Eyelid Deformity

Objective:To explore whether there is a correlation between qi-asthenia constitution and bags under the eyes,and to analyze the relevant influencing factors,to use body dialectics to help patients with bags under the eyes to carry out postoperative care and recovery,by recuperating and improving their qi-asthenia bias constitution,to reduce the influence of susceptibility factors,so as to obtain more satisfactory long-term results after youth pouch plastic surgery.To further explore the clinical experience of the treatment of youth pouch and lacrimal groove depression deformity in young patients with posterior orbital septum fat release and metastasis by conjunctival approach and correction of eyelid orbital sulcus deformity.Methods:The study was conducted by means of epidemiological survey and questionnaires distributed on the Internet platform,in September 2018 to December 2019,nanjing university of Chinese medicine affiliated hospital plastic surgery clinic consultation of the study was conducted in 175 patients with eye bags,statistical analysis of data,analysis of the correlation between qi deficiency constitution and youth pouch and influencing factors.A total of 67 cases of youth pouch patients received correction of eyelid orbital sulcus deformity by conjunctival approach were admitted to the affiliated hospital of nanjing university of traditional Chinese medicine from June 2017 to December 2019.There were 3 males and 64 females.The average age is 22 years old.All of them were diagnosed as lower eyelid youth pouch with different degrees of posterior orbital septum fat herniation without or with mild lower eyelid skin relaxation.Different degrees of orbital eyelid sulcus,bony orbital margin contour;Or suborbital hypoplasia.All cases were released via the conjunctival approach through the lower eyelid arcuate margin,and the posterior orbital septum fat flap was transferred to the osseous orbital margin and suborbital region and covered with fixation under local infiltration anesthesia.Because of the limited amount of fat in the posterior orbital septum,the posterior auricular fat fascia flap was transplanted into the orbital eyelid sulcus in 3 cases.The lateral tension of the lower eyelid margin is too large,which affects the lateral canthus Angle of the operative operator.Results:1.175 patients with youth pouch,the number of patients with qi-deficiency constitution was 145(82.9%),and the number of patients with non-qi-deficiency constitution was 30(17.1%).2.There were significant statistical differences in both types of patients,the data analysis results were P<0.01 in the aspects of personality tendency,genetic tendency and exercise habit.3.There was no statistical difference in menstrual index between qi-asthenic and non-qi-asthenic youth pouch patients.(P>0.05)4.All cases had no postoperative complications.The patients were followed up for more than 3 months after the operation,among which 3 cases were bilateral and 1 case was unilateral.In other cases,the youth pouch deformity and orbital sulcus of the eyelid were significantly improved,and the dynamic expression(orbicularis bicularis muscle contraction)showed no local uplift in the morphology of the lower eyelid,and the surgical effect was satisfactory.Conclusion:1.There is a correlation between qi-deficiency constitution and bags under the eyes.Qi-deficiency is one of the susceptibility factors for the formation of bags under the eyes.2.Posterior orbital septum fat release and metastasis via conjunctival approach and correction of eyelid orbital sulcus deformity can effectively improve the youth pouch deformity and eyelid orbital sulcus depression in young patients.The correct selection of indications is the premise of good clinical effect.
