| Objective:through the clinical investigation of coronary heart disease with depression,to analyze the related risk factors,and to explore the distribution and combination of TCM syndrome elements.In order to provide clinical reference for the prevention,diagnosis and treatment of the disease.Methods:From May 2019 to November 2019,194 inpatients with coronary heart disease in the Department of cardiovascular medicine of Jiangsu Hospital of traditional Chinese medicine were included consecutively.Collect the relevant data,use the Zung Depression self-assessment scale(SDS)to evaluate depression,fill in the four diagnosis information questionnaire of the selected subjects,use SPSS 17.0 statistical analysis software package to process the data,analyze the related risk factors of coronary heart disease with depression,and explore the syndrome distribution and combination rule of the disease,and extract the main syndrome types of coronary heart disease with depression by cluster analysis.Results:1.Among 194 patients with coronary heart disease,71 had depression,123 had no depression.The prevalence of coronary heart disease with depression was 36.60%.2.Multiple logistic regression analysis showed that female,obesity,cerebral infarction and heart failure were independent risk factors of coronary heart disease with depression.(or values were 2.178,3.082,2.392 and 3.141,respectively,P<0.05)3.The distribution of syndrome elements of coronary heart disease with depression is as follows:the syndrome elements of disease location are mainly heart and liver,accounting for 40.34%and 28.41%respectively.The main syndromes were qi stagnation,blood stasis,phlegm turbid and Qi deficiency,accounting for 20.20%,19.70%,17.73%and 16.75%respectively Compared with the patients without depression,the patients with depression had statistical significance(P<0.05)in Qi deficiency,qi stagnation and phlegm turbidity,and the distribution of other syndrome elements had no statistical significance(P>0.05).4.The combination of syndrome elements of coronary heart disease with depression is as follows:the combination of heart and liver is the most common,accounting for 30.99%;the disease combination is divided into single disease,double disease,three disease,four disease and above,of which three disease is the main,accounting for 50.70%;the combination of deficiency and real disease is divided into simple deficiency syndrome,simple empirical,and deficiency and real mixture,of which deficiency and real mixture is the main,accounting for 70.42%.5.The results of cluster analysis are divided into three categories:the first is Yin deficiency,fire(heat),Yang deficiency,blood deficiency and kidney;the second is Qi deficiency,phlegm turbidity and spleen;the third is heart,liver,qi stagnation and blood stasis.Combined with professional knowledge,the common syndrome types of coronary heart disease with depression can be summarized as qi stagnation and blood stasis,liver stagnation and qi stagnation,heart blood stasis,phlegm stagnation,spleen deficiency,yin deficiency and fire hyperactivity,kidney yang deficiency.Conclusion:1.coronary heart disease with depression has a high incidence rate.Female patients,obese patients and patients with cerebral infarction and heart failure are prone to depression.2.In the aspect of clinical syndrome differentiation,the syndrome differentiation characteristics of patients with coronary heart disease and depression are:1)multiple internal organs with the same disease,the main responsibility is on the heart and liver;2)the disease is mixed,mainly with deficiency and excess.At the same time,deficiency of Qi,stagnation of Qi and turbid phlegm are the important syndrome elements in the development of coronary heart disease with depression.3.The common syndromes of coronary heart disease with depression are qi stagnation and blood stasis,liver stagnation and qi stagnation,heart blood stasis,phlegm stagnation and block,deficiency of Spleen Qi,deficiency of yin and fire,deficiency of kidney yang. |
