Objective: To explore the main risk factors of stroke in recent years in China by means of meta-analysis.The harvard cancer risk index was used to evaluate the risk level of stroke,so as to accurately identify the high-risk population of stroke.This model was applied to the physical examination population in urumqi and the high-risk population of stroke was screened out,and the health literacy level and related influencing factors were analyzed,so as to provide strategic support for improving the primary prevention of stroke.Methods: This study was divided into three parts.First,the risk values of stroke risk factors were obtained by Meta analysis based on a search of literatures on risk factors for stroke published from January 2005 to October 2020.Second,the establishment and validation of the model.Based on the risk value of stroke selected from the meta-analysis,the harvard cancer risk index method was introduced to construct the risk rating model of stroke,and the ROC curve was used to verify its effectiveness.Third: this model was applied to the medical examination population in urumqi and screened out the high-risk population of cerebral apoplexy,and the health literacy level and related influencing factors were investigated and analyzed at the same time.Results: 1)meta-analysis results:a total of 53 articles were included in this study,including 50 case-control articles and 3cohort studies.In this meta-analysis,a total of 21 risk factors affecting the incidence of stroke were involved in the included literature,among which education level,physical labor and psychological stress showed no statistical significance.There were 18 risk factors with statistical significance,including 16 risk factors and 2 protective factors.The main indicators include lifestyle and dietary habits,physical examination biochemical indicators,physical examination indicators,disease history and family history.The results showed that these factors were the risk factors for stroke.2)build stroke risk indexassessment model and verification results: harvard cancer risk index prediction model specific as follows: according to the RR value and the size of the sample size in the Meta analysis,finally into the model of risk factors for hypertension,diabetes,CVHI,smoking,drinking,TG,LDL,TC,BMI > 24kg/m2,heart disease history,age,cerebrovascular disease family history.The results of this study showed that the average risk score for stroke was 28 points.Model validation results: 80 patients were diagnosed with stroke in the first affiliated hospital of xinjiang medical university,and 80 healthy control groups were matched,with a total of 160 patients.This model was verified and ROC curve was drawn.The area under the ROC curve of the stroke risk rating model was 0.730,95% CI(0.650~0.809).The results show that the model has good accuracy.3)application of the model and status of health literacy of high-risk groups of stroke the established risk assessment model of stroke was applied to 6,200 people who volunteered to receive cerebrovascular function test in the health management department of xinjiang medical university for physical examination,and 2,368 high-risk groups of stroke were screened out to investigate their level of health literacy and influencing factors.The results showed that age,ethnic distribution,marital status distribution and educational background distribution were the influencing factors for the health literacy level of the high-risk stroke population in urumqi.Conclusion: In this study,based on the meta-analysis,the harvard cancer risk assessment index was used to establish a risk assessment model for stroke incidence,and CVHI,TC,LDL and other objective indicators were included in the model in the form of assigned scores for the first time.Through verification,the prediction effect of this model was at a high level.This model was applied to 6,200 patients receiving CVHI in the health management department of xinjiang medical university from 2019 to 2020,and 2368 patients with high risk of cerebral apoplexy were screened out to investigate their health literacy level and related influencing factors.This study provides a basis and guidance for improving the screening and implementation of intervention measures for high-risk population of stroke in urumqi. |
