| ObjectionThe aim is to analyze the clinical manifestations,auxiliary examination and treatment of systemic lupus erythematosus associated Sweet syndrome,improve acquaintance of the two diseases and correlation.MethodsThe clinical data of 1 patient with sweet syndrome associated with systemic lupus erythematosus were analyzed retrospectively,and the related literatures were reviewed to discuss the clinical manifestations,pathogenesis and correlation of the two diseases.ResultsA 62-year-old female suffered from recurring painful red nodules on her face,neck and back;Double buccal edematous erythema,accompanied by mild desquamation;Hair loss,dry,and rough;Multiple oral ulcer;And joint swelling and pain in the extremities.Auxiliary examination results:WBC 2.81*10^9/L;ESR 40mm/h(normal:0-20mm/h)and complement C3<0.18(normal:0.9-1.8g/l),complement C4<0.064g/L(normal:0.1-0.4g/l);anti nuclear antibody 1:100(+),anti double stranded DNA determineation 125.10 IU/Ml(normal:0-100IU/ml),urine protein(+),24 h urinary protein quantitative 915.0mg/24h;alanine aminotransferase 207.9U/L(normal:7-40u/L),aspartate amino transferase 198.1U/L(normal:13-35U/L);Histopathological examination revealed: There was no obvious abnormality in epidermis,and there is marked neutrophils infiltrate within the dermis.Diagnosis:1.systemic lupus erythematosus 2.Sweet syndrome.ConclutionsNeutrophilic dermatose associated with autoimmune connective tissue disease is not uncommon,however,sweet syndrome has rarely been reported in systemic lupus erythematosus,which has increased the difficulty of diagnosis in some way.We report a case of sweet syndrome associated with systemic lupus erythematosus.Sweet syndromeis a neutrophilic dermatose,and the pathogenesis may be related to immunization and other factors.It may be independent or may be a special clinical manifestation of systemic lupus erythematosus.Diagnosis depends mainly on histopathological examination.It responses to small dose of glucocorticoid.We hope that dermatologists pay more attention to autoimmune connective tissue disease related Sweet syndrome.The relationship and mechanism between neutrophilic dermatosis and autoimmune connective tissue remains further research. |
