Objective:To compared and evaluated percutaneous balloon compression(PBC)and microvascular decompression(MVD)for primary treatment of trigeminal neuralgia(TN)in patients。Methods:In this retrospective comparative study,37 patients with TN were evaluated.Between January 2017 and December 2018,24 PBC procedures and 14 MVD procedures were performed. The therapeutic efficacy, operation time, inpatient time, complications and hospitalization expenses were compared between two groups。Results:Immediate pain relief occurred in 95.8%of patients treated with PBC and in 100%of patients treated with MVD.There were no statistically significant difference between the two groups(P>0.05).The operation time, hospital stay and hospitalization expenses in MVD group was longer than that in PBC group[(230±58.4)min vs.(42.8±17.3)min,(21.7±5.7)d vs.(9.8±3.6))d,all P<0.05),(41769.8±20355.65)yuan vs.(11395.2±2751.3)yuan,all P<0.05].The incidence of facial numbness was significantly greater in PBC group than MVD group(54.2%vs.0%,P<0.05).The incidence rate of postoperative complications in the MVD group was lower than that in the PBC group(P<0.05).One patient had cerebrospinal fluid leakage, one patient had subdural hematoma and two patient had intracranial infection in MVD.two patient had limitations in mandibular activity, two patient had hematoma of the cheek and one patient had diminished corneal reflex in PBC.Conclusion:MVD and PBC both are effective in treatment of primary trigeminal Neuralgia. The MVD is lower postoperative complications. The MVD is no facial numbness, limitations in mandibular activity and diminished corneal reflex. The PBC has the higher security, minimal injury, lower operation time、hospitalization expenses and inpatient time.
