Development And Application Of Oral Eveluation Form For Patients With Radiotherapy And Chemotherapy

Objects:To understand the current situation of oral evaluation of patients with radiotherapy and chemotherapy by nurses in oncology department;To determine the item pool for oral evaluation of radiotherapy and chemotherapy patients by Delphi expert correspondence,developing the oral evaluation tools for radiotherapy and chemotherapy patients;The scale was applied in clinic,reliability and validity test,further improved and modified to provide a scientific,accurate,sensitive,high-specificity,high-risk assessment tool for oral complications for nurses in the oncology department,so as to adopt reasonable preventive measures.Reduce the pain of patients and improve the quality of nursing.Methods:The study has three stages:In the first stage,198 nurses were selected from 7 general hospitals in Zhenjiang,Nanjing,Suzhou and other cities from September to December,2017,the present situation of oral evaluation of radiotherapy and chemotherapy patients was investigated;In the second stage,the oral evaluation tool was developed for 21 radiotherapy patients and 10 chemotherapy patients for one consecutive course of oral observation and 10 chemotherapy patients for two consecutive courses of oral observation,forming the original item of the evaluation tool,and through the patient interview,reference to domestic and foreign literature to perfect and supplement the original items,the formation of radiotherapy and chemotherapy patients oral assessment form entry pool.A total of 15 experts in the field of nursing management and nursing education were selected from the Cancer treatment Center of a tertiary first class hospital in Zhenjiang City who had been working for more than 10 years,and 15 specialists in the field of nursing management and nursing education were selected for two rounds of expert correspondence.The results of correspondence were collected,collated,analyzed statistically,and the item pool was modified to form a perfect oral evaluation tool for patients with radiotherapy and chemotherapy.In the third stage,141 cancer patients who were receiving radiotherapy,chemotherapy,radiotherapy and chemotherapy were investigated,and the reliability and validity of the scale were evaluated.Finally,the paper was written.Results:1 12.63% of the nurses evaluated the patients with primary radiotherapy and chemotherapy before the occurrence of oral complications,;97.98% nurses performed oral evaluation of tumor patients during oral complications;63.64% nurses made oral evaluation on patients with re-admission chemotherapy.The nurses in oncology department of 7 hospitals had different methods,contents and tools to evaluate patients’ oral cavity.It was considered that there was no uniform and targeted evaluation tool.2 Through interviews with cancer patients with radiotherapy and chemotherapy,oral observation,the reference literature formed a pool of oral evaluation items for patients with radiotherapy and chemotherapy,including 7 first-degree indexes and 26second-level indexes,which served as the first round of expert correspondence.The recovery rate of the first round of expert correspondence was 85%,the average index was 3.67-5.00,the standard deviation was 0.000-0.90,the coefficient of variation was 0.00-0.25,and the coordination coefficient of the expert was 0.422.In the second round,the recovery rate of expert correspondence was 100%,the average index was 3.87-5.00,the standard deviation was 0.000-0.83,the coefficient of variation(CV)was 0.000-0.21,and the coordination coefficient of expert was 0.437.The authority coefficient of two rounds of correspondence inquiry experts is 0.86,finally formed a total of 9 first-level indicators,35 second-level indicators;3 Reliability and validity of the scale:After 4 months of clinical application,the content validity of the final scale was 0.86.The total Cronbach’s α coefficient of the scale was 0.818,and the split-half reliability was 0.807.All of them were within an acceptable range;The clinical application of oral evaluation form of radiotherapy and chemotherapy showed that the incidence of oral complications in the control groupand the preventive group was statistically significant.Conclusions:1 Nurses did not pay enough attention to oral evaluation before and after admission and chemotherapy,the oral evaluation during radiotherapy and chemotherapy was lack of continuity,and the evaluation content during oral complications was lack of pertinence.There is also a lack of dedicated oral evaluation tools,currently clinical oral evaluation practice is not standardized;2 The oral evaluation scale of patients with radiotherapy and chemotherapy was developed by two rounds of expert correspondence method.There were 9 dimensions and 35 items.The results were scientific and reliable,which was helpful to the oral risk assessment of cancer patients.3 The oral evaluation scale for patients with radiotherapy and chemotherapy has good reliability and validity,can accurately reflect the oral condition of the patients,and provides a targeted assessment tool for clinical medical staff;Radiotherapy and chemotherapy in patients with oral assessment table for improving the oral environment and reducing oral complications have clinical significance.
