Objective: Research the value of Frailty score in the prognosis of elderly multiple myelomaMethods: Retrospective analyzed the clinical and laboratory data of 183 elderly patients(age ≥ 65 years)with MM who were diagnosed and treated in the Department of Hematology,Shengjing Hospital,China Medical University from January 2012 to March 2018,including age,Gender,ISS stage,DS stage,M protein typing,survival time,HB,WBC,PLT,serum creatinine,serum calcium,serum albumin,LDH,β2 microglobulin and bone marrow plasma cell ratio,Daily Living Ability Table(ADL),the Instrumental Daily Living Activity Table(IADL)and the Charlesson Comorbidity(CCI)score.A Frailty score based on the Old Age Assessment Criteria(CGA)was applied to all patients and divided into healthy group(0 points),moderate group(1 point),and weak group(≥ 2 points).Survival analyses were carried out for three groups with different health conditions.Results: DS stage,LDH level,bone marrow plasma cell ratio,ADL score,IADL score,and CCI score all affected the prognosis of MM patients(p<0.05),whereas age ≥75,CCI ≥2,and ADL≤4 ware independent risk factors for prognosis of MM patients(P<0.05).The median survival time of the healthy group was 65 months,moderate group was 47.8 months and weak group was 24.4 months.The survival time of the healthy group was significantly longer than that of the other groups(P<0.05).Patients in the moderate and weak groups had a greater risk of death and a shorter median survival time than patients in the healthy group.The 3-year survival rate was 44.44% in the healthy group,53.85% in the moderate group,and 5.19% in the weak group.There was a statistical difference in the three groups(P<0.001).The 3-year survival rate of the healthy group and the moderate group was significantly higher than that of the weak group.Conclusion: The Frailty scoring system is of great significance for the prognosis of elderly patients with myeloma.The survival in the healthy group was significantly longer than that in the moderate and weak groups,and the 3-year survival rate was higher.Patients in the moderate group and the weak group had a higher risk of death.
