The Influence Factors Of Pulmonary Compliance After Tracheal Intubation In Pediatric Patients

Objective:Good lung compliance is a necessary condition to maintain perioperative respiratory function.Along with scientific research and clinical trials on the lung compliance,the influence of many factors on lung compliance in perioperative period are causing more and more people's attention.This research aims to explore the influence factors of pulmonary compliance after tracheal intubation in paediatric patients undergoing adenotonsillectomy,and the effect of preoperative administration of atropine or endotracheal suction after endotracheal intubation on pulmonary compliance in children.METHOD:The study population consisted of 145 children ASA 1~2 scheduled to undergo adenotonsillectomy.Of these,the children were allocated to one of the three groups: the control group(A)? the atropine group(B)?the suction group(C).Atropine group received premedication of atropine 0.01mg/kg intravenous just before general anesthesia induction;Suction group received an endotracheal suction at 10 minutes after endotracheal intubation?Airway resistance(Raw)and dynamic lung compliance(Cdyn)were determined 5,10,15 min after tracheal intubation and compared with baseline and between the three groups.RESULT:According to the data at 5 min after tracheal intubation,age,height,weight and pulmonary auscultation are correlated with Cdyn and Raw.Children with atropine treatment displayed a higher Cdyn and a lower Raw contrasted with control group at 5 min,10 min and 15 min,and the same results was observed of children with Suction treatment at 15 min,further more,the suction group had no statistic difference compared with the control group on Cdyn and Raw at 5 min.CONCLUSION:Age,height,weight and pulmonary auscultation are all correlated with children's Cdyn and Raw after tracheal intubation,and age shows strongest correlation with Cdyn and Raw among all influencing factors of Cdyn and Raw.Endotracheal suction after endotracheal intubation can increase Cdyn and decrease Raw.Endotracheal suctioning after endotracheal intubation were advantageous.
