As the concept of joint decision-making between patients and doctors is put forward and promoted gradually,nurses’ decision-making is increasingly prominent.Previous studies have proved that nurses’ participation in medical decision-making has significance to patients’ better understanding treatment strategies,which can enhance the treatment effect and improve patients’ satisfaction.Recently,as an important variable of individual difference,regulatory mode has been widely concerned by researchers in the field of decision-making.Regulatory mode refers to the psychological behavior strategy adopted by individuals in the process of self-regulatory,which is a relatively stable personality trait variable.A large number of studies have confirmed that there is a direct causal relationship between regulatory mode and decision-making.However,in the medical field,the relationship between regulatory mode and risk decision-making is still unknown.Especially the relevant research on the nurse group has not been carried out.The characteristics of nurses’ regulatory mode and the differences of nurses’ medical decision-making in different regulatory modes are worth exploring deeply.Based on this,in this study we chose nurses as the research object,"regulatory mode" and "medical risk decision-making" as the cut-in point,and based on the two perspectives of nurses’ decision-making for themselves and for others to empirically analyze the influence of trait and situational regulatory modes on nurses’ medical risk decision-making and the mediating effect of anticipated regret.What’s more,the characteristics of nurses’ self-other medical risk decision-making was further investigated.The aim of this study was to help nurses better understand their own decision-making characteristics and improve their clinical decision-making ability.Moreover,the purpose of the study was to provide scientific basis for reducing decision-making errors and optimizing decision-making quality in the medical decision-making process.Objective1.To explore the influence mechanism of trait regulatory mode and situational regulatory mode on nurses’ medical risk decision-making.2.To analyze the characteristics of nurses’ self-other medical risk decision-making.Methods1.With “Regulatory Mode Scale”,“anticipated regret questionnaire” and “medical risk decision-making questionnaire”,we carried out our study on 498 nurses from two grade A class 3 hospitals in Xi’ an.We used multiple linear regression analysis and the mediating effect analysis to explore the influence of trait regulatory mode on nurses’ medical risk decision-making,and to examine the mediating effect of anticipated regret.2.With “medical risk decision-making questionnaire” and “anticipated regret questionnaire” as evaluation tools and using “memories-writing task” to manipulate situational regulatory mode we carried out the study on 198 nurses from a grade A class 3 hospital in Xi’ an.We used t-test,multiple linear regression analysis and the mediating effect analysis to explore the mediating effect of anticipated regret between situational regulatory mode and medical risk decision-making.3.With “Regulatory Mode Scale” and “medical risk decision-making questionnaire” we carried out a study on 505 nurses in Xi’an.We used t-test and two-factor anova to explore the difference between nurses’ decision-making for themselves and for others,and to analyze the influence of regulatory mode.Results1.The results of the influence of the two types(trait and situational)of regulatory mode on nurses’ medical risk decision-making were consistent.1.1 The framing effect of nurses’ medical risk decision-makingIn the positive framing,the majority(79.8%;77.0%)of nurses chosed the safe program,and the score of medical risk decision-making was significantly lower than 3.5(p<0.05),suggesting that nurses tended to risk aversion.In the negative framing,most(70.9%;62.2%)nurses chosed risky program and the score of medical risk decision-making was significantly higher than 3.5(p<0.05),suggesting that nurses tended to risk seeking.The results were consistent with Kahneman’s results.1.2 Multiple regression analysis of the influence of regulatory mode and anticipated regret on nurses’ medical risk decision-making in different framingLocomotion mode positively predicted the medical risk decision-making(p<0.05),assessment mode negatively predicted the medical risk decision-making(p<0.05),and anticipated regret negatively predicted the medical risk decision-making(p<0.05)in both positive framing and negative framing.1.3 The mediating effect of anticipated regret on the relationship between regulatory mode and medical risk decision-making in different framingThe mediating effect of anticipated regret was significant in different framing.(1)According to study 1,in the positive framing,the indirect effect of locomotion mode on nurses’ medical risk decision-making accounted for 17.86% of the total effect.The indirect effect of assessment mode on nurses’ medical risk decision-making accounted for 13.33% of the total effect.In the negative framing,the indirect effect of locomotion mode on nurses’ medical risk decision-making accounted for 17.14% of the total effect;the indirect effect of assessment mode on nurses’ medical risk decision-making accounted for 18.75% of the total effect.(2)According to study 2,the indirect effect of regulatory mode on nurses’ medical risk decision-making accounted for 28.95% of the total effect in the positive framing.The indirect effect of regulatory mode on nurses’ medical risk decision-making accounted for 31.71% of the total effect in the negative framing.2 The influence of locomotion mode on nurses’ self-other medical risk decision-making2.1 Results of nurses’ self-other medical risk decision-making in different framingIn the positive framing,the score of nurses’ medical risk decision-making for others was significantly lower than that of decision-making for themselves,and the difference was statistically significant(p<0.05).Moreover,the score of decision-making for others was significantly lower than 3.5(p<0.05),suggesting that the nurses tended to risk aversion.But there was no statistically significant difference between the score of self decision-making and 3.5(p>0.05),suggesting that the tendency of risk aversion and risk seeking was almost consistent.In the negative framing,the scores of nurses’ decision-making for themselves and for others were both higher than 3.5(p<0.05),suggesting risk seeking.And the difference between decision-making for themselves and for others was no statistically significant(p>0.05).2.2 The interaction between regulatory mode(locomotion/ assessment)and self-others was significant in different framing.(1)In positive framing:(1)For the nurses of locomotion mode,the score of decision-making for themselves was significantly higher than that of decision-making for others,and the difference was statistically significant(p<0.05).When decision-making for themselves,the score was significantly higher than 3.5(p<0.05),suggesting risk seeking.When decision-making for others,the score was significantly lower than 3.5(p<0.05),suggesting risk aversion.(2)For the nurses of assessment mode,the difference between the score of decision-making for themselves and for others was no statistically significant(p>0.05).And compared with 3.5,the scores of decision-making for others and for themselves were both significantly lower(p<0.05),suggesting risk aversion.(2)In negative framing:(1)For the nurses of locomotion mode,the scores of decision-making for themselves and for others were both significantly higher 3.5(p<0.05),suggesting risk seeking.And the score of decision-making for themselves was significantly higher than that of decision-making for others(p<0.05),suggesting that nurses tended to be more risk-seeking when decision for their own.(2)For the nurses of assessment mode,the difference between the score of decision-making for themselves and for others was no statistically significant(p>0.05).And compared with 3.5,both were no statistically significant(p>0.05),suggesting that risk seeking and risk aversion were basically consistent.Conclusion1.There is a framing effect in nurses’ medical risk decision-making.In the positive framing,they tend to risk aversion,while in the negative framing,they tend to risk seeking.2.Regulatory mode has an important impact on nurses’ medical risk decision-making.Locomotion mode positively predicts medical risk decision-making,and assessment mode negatively predicts risk decision-making.Besides,anticipated regret plays a mediating role between the regulatory mode and risk decision-making.3.In the positive framing,nurses are more risk-averse when making decision for others than when making decisions for themselves.In the negative framing,nurses tend to risk seeking for others decision-making as well as for their own.4.The interaction between regulatory mode(locomotion/ assessment)and self-other was significant in different framing.In the positive framing,locomotion-oriented nurses tend to be risk seeking when decision-making for themselves while tend to be risk aversion when decision-making for others.In the negative framing,nurses’ decision-makings for themselves and for others both tend to risk seeking,and the tendency of risk seeking is stronger when self decision-making. |
