| Objective To investigate the relationship between serum BDNF and severity and prognosis in patients with traumatic brain injury.Method TBI patients with eligible conditions were included in the study(ie TBI group),healthy controls were included in patients without TBI,without stroke, encephalitis, brain tumor or active cancer, and without demyelinating disease, dementia, schizophrenia Patients with neurodegenerative diseases such as age(age 18-70 years). The TBI group was based on the GCS score at admission(subjective assessment of the response of the patient’s eyes, language, and limbs to different stimuli): the mild TBI group was defined as 12-15 points, moderate TBI group was defined as 9-12 points, severe TBI was defined as 3-8 points, that is,the lower the score, the more severe the condition;the good prognosis group and the poor prognosis group were grouped according to the GOS score after 3 months(subjective assessment by the patient’s neurological recovery): That is,the good prognosis group and the poor prognosis group, and the low scores of the evaluation also indicate that the prognosis is relatively poor.We use 1-3 points and 3-5 points for classification.Collect Serum samples after admission at 1,7,and 14 day, then detect the level of BDNF by ELISA method. Results 1.Compared the levels of serum BDNF at different time points with the control group, the level of serum BDNF at the 1st day was not significantly different from that of the control group(P>0.05).The other groups were significantly different from the control group at different time points(P<0.01).2.The mean serum BDNF levels were not the same at 1,7,and 14 day in the group at different time points(P<0.01).Time interacts with mild, moderate, and severe TBI groups. The serum BDNF levels at different time points in the mild, moderate, and severe TBI groups were compared with each other(P<0.01),which was statistically significant.3.The levels of serum BDNF decreased at 1d after injury, and gradually increased after 7d.The mild TBI group was always higher than the moderate TBI group at each time point, and the moderate TBI group was always higher than the severe TBI group.4.The lower the serum BDNF level, the more serious the condition.5.Combined with the GOS of 3 months after injury, the serum BDNF level in the good prognosis group was higher than that in the poor prognosis group at 1,7,and 14 day in the group at different time points.6.Through correlation analysis, the lower the serum BDNF level, the lower the GCS and the GOS after 3 months of injury. Conclusions As a protein biomarker of the central nervous system,The serum BDNF level is closely related to the severity of TBI after acute injury,It is important to evaluate the serum BDNF level in patients with acute injury at the special injury time or at the special time point during hospitalization, and to guide the patient’s recovery and final outcome in combination with the change of the condition. |
