| Objective: Lung cancer is the most frequently diagnosed cancer in the world and the leading cause of cancer death.This paper intends to study the association between serum tumor markers and plasma coagulation function changes in patients with lung cancer and explore its diagnostic value and clinical significance for lung cancer.Methods: 157 lung cancer patients,56 subjects with benign lung disease(BLD)and 40 healthy volunteers were included in this retrospective study.The levels of serum carcinoembryonic antigen(CEA),neuron specific enolase(NSE),cytokeratin19fragments(CYFRA21-1)were detected by electrochemiluminescence immunoassay and plasma fibrinogen(FIB),d-dimer(D-D)were measured by immunoturbidimetric assays.Platelet(PLT)was determined by resistance method and these results were statistically analyzed.Receiver operating characteristic(ROC)curve was performed and the area under the curve(AUC)was calculated to evaluate the diagnostic value of each indicators.Results:1.CEA,NSE,CYFRA21-1 and FIB,D-D,PLT levels in lung cancer patients were statistically significant compared with BLD and healthy control group(P<0.05).2.There was no statistically significant difference in CEA,PLT,D-D levels and positive rate between different histopathological types(P>0.05).CYFRA21-1 level and the positive rate in squamous cell carcinoma(SCC)group were higher than that of adenocarcinoma(ADC)and small cell lung cancer(SCLC)group(P<0.05).The NSE level and the positive rate in SCLC group were higher than that of ADC and SCC group(P<0.05)and FIB level and positive rate in SCC and SCLC group were higher than in ADC group(P <0.05).3.All indexes levels were elevated in high stage(III/IV)than in lower stage(I+II)(P<0.05),except for PLT between high stage(III/IV)and lower stage(I+II)(P>0.05).The CYFRA21-1 level in IVB stage lung cancer group were higher than that in IVA period(P< 0.05).4.The above six indexes in the lung cancer with lymph node metastasis group were significantly higher than those without metastasis group(P<0.05).5.The plasma FIB level were positively correlated with CYFRA21-1(P< 0.01),and the correlation factor r was 0.274.D-D level were positively related to CEA,NSE,CYFRA21-1(P< 0.05),and the correlation factor r was 0.20,0.29 and 0.33,respectively.6.The sensitivity and specificity of CEA for lung cancer was higher than the other markers,which was 51.6% and 87.5% respectively.The sensitivity(89.2%)and negative predict value(64.6%)of six kinds of markers jointed detection in lung cancer and AUC(0.832)in NSCLC were higher than that of every single marker.Conclusions: CEA,NSE,CYFRA21-1 and FIB,D-D,PLT have certain correlation and have certain value in the pathological types,clinical stages and lymph node metastasis of lung cancer.The joint detection of them can improve the sensitivity for lung cancer. |
