Hemangioblastoma is a relatively rare tumor of CNS,which often occurs in the cerebellum and progresses slowly.It belongs to WHO Ⅰ grade tumors.The morphology characteristics included ample capillary network and foamy stromal cells,forming typical “clear cell” morphology.So it should be differentiated from metastatic renal clear cell carcinoma,angiomatous meningioma,pilocyticastrocytoma.Mesenchymal cells are the essential components of hemangioblastoma which their origin remains unclear.L1CAM(neural adhesion molecule),namely CD171,is one of the important members of the cell surface immunoglobulin family and plays an important role in the development of the nervous system.Recent studies reported that LICAM was constitutively and strongly expressed in neuroectodermal tumors such as schwannomas and neuroblastoma.However,the significance of L1 CAM in hemangioblastoma has not been reported.Galectin-3,β-galactoside-binding protein,is a member of the lectins family and is mainly used for the diagnosis of papillary thyroid carcinoma and follicular carcinoma.Objective: Our purpose is to study the expression of L1 CAM and galectin-3 in hemangioblastoma,and the significance of diagnosis and differential diagnosis in hemangioblastoma.Methods: 30 cases of hemangioblastoma paraffin specimens,10 cases of renal clear cell carcinoma,9 cases of angiomatous meningioma,4 cases of pilocyticastrocytoma,8 cases of central neurocytoma,6 cases of schwannomas,4 cases of malignant peripheral nerve sheath tumor,9 cases of pheochromocytoma were selected from the Department of Pathology,the First Affiliated Hospital of China Medical University from 2013 to 2017.The expression of L1 CAM and galectin-3 was detected by immunohistochemistry.Results: The positive expression rate of L1 CAM in hemangioblastoma and renal clear cell carcinoma,angiomatous meningioma and pilocyticastrocytoma was low.L1 CAM is diffusely and positively expressed in central neurocytoma.The positive expression rate of galectun-3 in hemangioblastoma was significantly higher than that of renal clear cell carcinoma,angiomatous meningioma and pilocyticastrocytoma,with significant differences.Conclusion: L1 CAM has no significance in the diagnosis of hemangioblastoma.Hemangioblastoma is neither a tumor of neuroectodermal origin or neuronal differentiation.In addition,Galectin-3 can be used as a specific diagnostic marker for hemangioblastoma,and can be used to guide the differential diagnosis of metastatic renal clear cell carcinoma,angiomatous meningioma and pilocyticastrocytoma.
