Effects Of Risperidone Combined With Health Education On Mood And Quality Of Life In Patients With First-episode Schizophrenia With Different Duration Of Untreated Psychosis

ObjectiveHealth education is the most widely used intervention mode in clinical practice.It is beneficial to improve the drug compliance of patients,promote disease rehabilitation and return to society,and improve the quality of life.However,some patients have poor response to health education.What is the cause of this phenomenon? There are few reports at present.The purpose of this study was to explore the effects of risperidone combined with health education on mood and quality of life of patients with first-episode schizophrenia in different untreated periods(DUP).Methods98 patients with first-episode schizophrenia were divided into observation group and control group by random number table method,49 cases in each group.According to the median of DUP(11 months),49 patients in the observation group were divided into long DUP group(n = 25)and short DUP group(n = 24).The patients in control group were treated with risperidone,while the patients in observation group were treated with risperidone combined with health education.Before treatment and 12 weeks after treatment,the positive and negative symptoms scale(PANSS),Hamilton Depression Scale-24(HAMD),Hamilton Anxiety Scale(HAMA)and Health Questionnaire(SF-36)were used to evaluate psychiatric symptoms,depression,anxiety and quality of life.Results:1.After 12 weeks of treatment,the PANSS positive symptoms,negative symptoms,psychopathological symptoms and total scores of the observation group and the control group decreased significantly(P < 0.05),and there was no significant difference between the two groups(P > 0.05).Before treatment,the negative symptoms of long DUP group were higher than those of short DUP group(P < 0.05),suggesting that the longer DUP was,the more serious the negative symptoms of first-episode schizophrenia were.There was no significant difference in the total score of PANSS scale and the reduction of each factor between the two groups before and after treatment(P > 0.05).2.After 12 weeks of treatment,the HAMD score and HAMA score of the observation group and the control group decreased significantly,and the observation group was significantly lower than the control group,the difference was statistically significant(P < 0.05).Before treatment,HAMA score of long DUP group was significantly higher than that of short DUP group(P < 0.05);HAMD score of long DUP group was also significantly higher than that of short DUP group,but there was no significant difference(P > 0.05).The results indicated that the longer DUP was,the more serious anxiety symptoms of first-episode schizophrenia patients were.The scores of HAMD and HAMA in long DUP group before and after intervention were lower than those in short DUP group(P < 0.05),suggesting that health education is more conducive to the improvement of depression and anxiety symptoms in short DUP patients.3.After 12 weeks of treatment,the total scores of physiological function,physiological function,social function,emotional function,mental health and SF-36 in the observation group were significantly higher than those in the control group(P < 0.05).The total scores of physiological function,mental health and SF-36 in the control group were significantly higher than those in the control group(P < 0.05).The total scores of physiological function,social function,emotional function,mental health and SF-36 in the observation group were higher than those in the control group(P < 0.05).The scores of social and emotional functions in long DUP group were lower than those in short DUP group before treatment(P < 0.05).The scores of physiological function,social function,mental health and total score of SF-36 in long DUP group before and after intervention were lower than those in short DUP group(P < 0.05),suggesting that health education is more helpful to improve the quality of life of short DUP patients.Conclusion:1.The negative symptoms,anxiety and quality of life of long DUP patients with first-episode schizophrenia are more serious/worse than those of short DUP patients.2.Health education may improve the quality of life of patients with depression and anxiety,especially for patients with short DUP.
