| Objective To explore the value of Raman spectroscopy of peripheral plasma and erythrocyte in the diagnosis of geriatric cognitive disorder.Methods 62 cases(aged>65years)from July 2016 to September 2016 in the Department of Geriatrics were analyzed.According to the assessment of neuropsychological scales,all cases were divided into three groups: control groups(group C),mild cognitive impairment group(group M)and dementia group(group D).The data of patients’ age,education years,the underlying diseases,lab examination indexes were collected and recorded.Whole blood of limosis were collected in sodium citrate anticoagulation tubes by venipuncture in the morning.Plasma and erythrocyte collected through centrifugal were stored at-80℃ till analyzed by lab of spectrum detection and instrument of Shanghai Jiao Tong Univers ity spectroscopy department.five-order polynomial fitting algorithm and discriminant analys is were applied to extract the effective data of blood samples and build data model.Value of diagnosis was evalauted by receiver operating characteristics curves.Results1.There are 62 cases(group C: 23 cases;group M: 23 cases;group D: 16 cases).There is no statistically significant discrepancies in sex,age and education years(p>0.05).Compare of underlying diseases(hypertension,coronary atherosclerotic heart disease,dyslipidemia and diabetes)among three groups,no significant statistical differences are observed.(p > 0.05).2.Lab examination indexes of all cases included white blood count(WBC),hemoglobin(HGB),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin Concentration(MCHC),red cell distribution width(RDW),hematocrit(HCT),serum albumin(ALB),serum pre-albumin(PA),creatinine(CR),blood uric acid(UA),alanine aminotransferase(ALT),aspartate aminotransferase(AST),Total cholesterol(TC),Triglycerides(TG),High-density lipoprotein cholesterol(HDL-C),Low-density lipoprotein cholesterol(LDL-C),cyscatinC(Cys C).The all indexes differences among three groups have no significant statistical differences(p > 0.05).3.With surface enhanced Raman spectra data of plasma,discriminant model between group C and group D can distinguish correctly 87.2% of cases.The area under the ROC curve(AUC)is 0.938 in ROC analys is,the corresponding sensitivity and specificity were 95.5% and 87.5% respectively;76.1% of cases can be distinguished correctly by discriminant model of group C and group M.AUC is 0.837,the corresponding sensitivity and specific ity were 87.0% and 78.3% respectively;Discriminant model of M group and group D can distinguish correctly 71.8% of cases,AUC is 0.788,the corresponding sensitivity and specificity were 75.0% and 78.3% respectively.4.Differences of surface enhanced Raman spectra of erythrocytes among groups are observed.With the surface enhanced Raman spectroscopy data of erythrocyte,discriminant model of group C and group D can distinguish correctly 71.8% of cases;The corresponding AUC is 0.791 in ROC analys is,the corresponding sensitivity and specificity were 91.3% and 56.2% respectively.58.7% of cases are distinguish by Discriminant model of Group C and group M correctly.AUC is 0.654,the corresponding sensitivity and specificity were 56.5% and 73.9% respectively.Discriminant model of Group D and group M can distinguish correctly 61.5% of cases.The AUC is 0.717,the corresponding sensitivity and specificity were 75.0% and 65.2% respectively.Conclusions1.Surface-enhanced Raman spectroscopy analysis of plasma not only has a certain value for the diagnosis of AD from normal people,but also can effectively distinguish normal people and patients with MCI.It’s helpful for early identification of cognitive dysfunction,which may have a high clinical application value.2.Discrepancies of surface enhanced Raman spectra of erythrocyte among different cognitive groups can be observed,which may have certain inspiration to us for studing of erythrocyte pathological physiology for AD.Compared with the plasma surface enhanced Raman spectroscopy,its diagnostic value in patients with cognitive dysfunction is low,which remains to be further mining. |
